消化性溃疡470例的胃镜特点分析

Analysis of the characteristics with gastroscope in 470 cases of peptic ulcer

目的了解不同地区消化性溃疡的发病特点。方法对我院6年内经胃镜检查诊断为消化性溃疡的病例进行统计分析。结果共检出消化性溃疡470例,其中十二指肠溃疡292例,胃溃疡153例,复合型溃疡25例,患者男女之比3.35∶1,十二指肠溃疡患者平均年龄40.1岁,胃溃疡患者平均年龄45.9岁。夏季发病率较秋冬和冬春低,溃疡直径<1 cm 330例,1~2 cm118例,>2 cm 22例;幽门螺旋杆菌的阳性率为73.56%。患者临床表现以典型腹痛为主,并发症中出血占24.04%。结论消化性溃疡中十二指肠溃疡发生率高于胃溃疡,男性高于女性;十二指肠溃疡较胃溃疡发病早,十二指肠溃疡病变部位以球前壁和球大弯为高发,胃溃疡以胃窦和胃角为高发。溃疡以小溃疡为主,巨大溃疡很少见,幽门螺旋杆菌感染是引起溃疡的主要原因,老年人腹痛不典型,出血仍是溃疡最多见的并发症。

Objective To clarify the characlerislics of peptic ulcer in differenl regions. Methods Cases of peptic ulcer diagnosed with gastroscopy in our hospilal in 6 years were analyzed slalislically. Results A lolal of 470 cases of peptic ulcer were delecled, including 292 cases of duodenal ulcer, 153 cases of gaslric ulcer, 25 cases of compound ulcer, and lhe ralio of male lo female was 3.35：1. The average age of palienls with duodenal ulcer was 40.1 years old, while lhe average age of lhe palienls with gaslric ulcer was 45.9 years old. The incidence rale in summer was lower than lhal in winter and spring. The ulcerdiamelerwaslesslhan1 cmin 330 cases,1？2 cmin 118cases, andmorelhan2 cmin 2 2 cases, andlheposiliverale of helicobacler pylori was 73.56%. Clinical manifestations were mainly abdominal pain, hemorrhage accounted for 24.04% of lhe complications. Conclusion The incidence of duodenal ulcer is higher than lhal of gaslric ulcer, and lhe incidence rale in male is significantly higher than lhal of lhe female. The incidence of duodenal ulcer is earlier than gaslric ulcer, lesions of duodenal ulcer in front of lhe ball wall and lhe greater curvature of lhe ball is high, gaslric ulcer in lhe gaslric anlrum and gaslric angle is high. Small ulcer is mainly in ulcer, and huge ulcer is very rare. Helicobacler pylori infection is lhe main cause of ulcer, abdominal pain in lhe elderly is nol typical, bleeding is slill lhe most common complications of ulcer.
