延续护理干预对老年糖尿病患者血糖控制及自我管理能力的影响

Influence of continuing nursing intervention on the glycemic control and self-management skill in aging patients with diabetes

目的探究延续护理干预对老年糖尿病患者血糖控制及自我管理能力的影响。方法选取2015年1月至2016年5月我院收治的老年糖尿病患者220例,随机分为对照组和观察组,每组110例。对照组实施常规护理干预,观察组在此基础上予以延续护理干预。比较两组患者血糖控制情况和自我管理能力水平。结果两组患者干预前血糖指标水平比较,差异无统计学意义(P>0.05);干预后,两组患者的2 h血糖、空腹血糖、糖化血红蛋白水平均显著降低,且观察组上述指标明显低于对照组(P<0.05);观察组患者饮食控制、自我监测、遵嘱服药、血糖异常处理、运动适量评分均明显优于对照组(P<0.05)。结论老年糖尿病患者实施延续护理效果显著,能有效维持患者血糖平稳,提高患者自我管理能力,值得临床推广与应用。

Objective To invesligale the influence of conlinuing nursing inlervenlion on the glycemic conlrol and self-management skill in aging patients with diabetes. Methods A lolal of 220 cases of senile diabetes patients admitted in our hospilal from January 2015 lo May 2016 were selecled and randomly divided inlo conlrol group and observalion group, wilh 110 cases in each group. The conlrol group was given routine nursing inlervenlion, and lhe observation group was given conlinuing nursing inlervenlion on lhe basis of lhe conlrol group. The situation of glycemic conlrol and self-managemenl skills of lhe lwo groups were compared. Results There were no significant differences in lhe levels of blood glucose indexes between lhe lwo groups before inlervenlion （ P 〉0.05）. After inlervenlion, lhe levels of blood glucose al postprandial lwo hours, fasting blood glucose and glycosylated hemoglobin of lhe lwo groups significantly descended and lhe above indexes in lhe observalion group were significantly lower than those of lhe conlrol group （ P〈0.05）. Compared wilh lhe conlrol group, lhe scores of diet control, self -monitoring, self administration, palhoglycemia handling and moderate exercise in lhe observalion group were significantly belter （ P〈0.05）. Conclusion Conlinuing nursing inlervenlion has significant effect on aging palienls wilh diabetes and which can effectively maintain stable blood glucose and increase self-managemenl skill, il is worthy of clinical promotion and application.
