达芬奇机器人辅助下乳腺癌带蒂大网膜乳房填充重建术

Da Vinci robot-assisted filling with pedicled omental flap for breast reconstruction

目的探讨达芬奇机器人辅助下乳腺癌带蒂大网膜乳房填充重建术的临床疗效,为乳腺癌患者乳房重建提供一个新的选择。方法 2017年3月我院收治1例Luminal型左乳癌女性患者,在全身麻醉下行左乳癌局部扩大切除术、左腋窝前哨淋巴结活检术及达芬奇机器人辅助下带蒂大网膜填充术。术后评估伤口血供和愈合情况、美容效果、并发症及患者满意度。结果患者术后恢复良好,伤口位置隐蔽;重建乳房形态良好,手感自然,双乳对称,美容效果良好;患者满意度高。术后随访2个月,患者未出现局部复发或转移。结论达芬奇机器人辅助下乳腺癌带蒂大网膜乳房填充重建术具有高清三维的手术视野、安全精准的手术操作,高度灵活的机械关节等优点,患者乳房自体重建后效果好,且对患者的心理健康和社会功能等有明显改善。

Objective To explore the effect of pedicled omental flap with Da Vinci surgical system for breast reconstruction,so as to fa-cilitate the application of robot-assisted surgery in the breast reconstruction ater breast cancer（BC） surgery- Metliods A luminal BC pa-tient, administrated in March 2017, received Da Vinci robot-assisted filling with pedicled omental flap ater extensive lumpectomy and sentinel lymph node biopsy. Blood supply,wound healing,cosmetic outcome,complications and satisfaction of the patient were evaluated. Results The wound healed well,with no obvious scar. The reconstructed breast had desirable volume and looked symmetrical The cosmetic outcome was excellent and the patient was satisfied with the surgery. No recurrence or metastasis was found during the two-montli post-operational follow-up. Conclusion Da Vinci robot-assisted breast reconstruction witli pedicled omental flap haclear vision,precise movement and flexible machinery joint. Breast conservation and autologous reconstruction with thimproves the psychological and social well-being of the patient.
