丙酸氟替卡松鼻喷剂联合电针治疗常年性变应性鼻炎效果观察

Efficacy of Fluticasone Propionate Nasal Spray Combined with Electroacupuncture in Treatment of Patients with Perennial Allergic Rhinitis

目的探讨丙酸氟替卡松鼻喷剂联合电针治疗常年性变应性鼻炎的临床效果。方法选取2014年1月—2015年5月凉山彝族自治州第一人民医院收治的符合纳入及排除标准常年性变应性鼻炎620例,随机将其分成观察组和对照组两组各310例,观察组采用丙酸氟替卡松鼻喷剂联合电针治疗,对照组仅采用丙酸氟替卡松鼻喷剂治疗,观察比较两组治疗前及治疗1年后免疫指标(T淋巴细胞亚群CD3+、CD4+、CD8+)、鼻阻力(NAR)、鼻腔黏膜纤毛清除功能(糖精清除时间)、鼻腔分泌物嗜酸粒细胞(EOS)计数检测结果,以及治疗1年后临床效果。结果治疗前,两组T淋巴细胞亚群CD3+、CD4+、CD8+水平及NAR、糖精清除时间、鼻腔分泌物EOS计数比较差异均无统计学意义(P>0.05)。治疗1年后,T淋巴细胞亚群CD8+水平、NAR、糖精清除时间及鼻腔分泌物EOS计数两组均较治疗前降低,且观察组均低于对照组,差异均有统计学意义(P<0.05);T淋巴细胞亚群CD3+、CD4+水平两组则均较治疗前升高,且观察组高于对照组,差异亦均有统计学意义(P<0.05)。治疗1年后,观察组治疗总有效率91.89%明显高于对照组治疗总有效率74.67%,差异具有统计学意义(P<0.05)。结论丙酸氟替卡松鼻喷剂联合电针治疗常年性变应性鼻炎效果优于单纯丙酸氟替卡松鼻喷剂治疗,利于患者病情改善。

Objective To investigate clinical effect of Fluticasone Propionate Nasal Spray combined with electroacupuncture in treatment of patients with perennial allergic rhinitis. Methods A total of 620 patients with perennial allergic rhinitis,who had met the inclusion and exclusion standard during January 2014 and May 2015,were randomly divided into observation group and control group （ n = 310 for each group） . Observation group was treated with Fluticasone Propionate NasalSpray combined with electroacupuncture,while control group was treated with Fluticasone Propionate Nasal Spray alone.Changes of immune indexes T lymphocyte subsets （ CD3 ＋ ,CD4 ＋ ,CD8 ＋ ） ,nasal resistance （ NAR） ,nasal mucociliaryclearance function （ saccharin clearance time）and eosinophil （ EOS）count in nasal secretions before and 1 year after treatment,and curative effects 1 year after treatment were observed and compared between the two groups. Results Before treatment,there were no significant differences in levels of T lymphocyte subsets （ CD3 ＋ ,CD4 ＋ ,CD8 ＋ ） ,NAR,saccharinclearance time and EOS count in nasal secretions between the two groups （ P 〉 0 05） . One year after treatment,levels of Tlymphocyte subset CD8 ＋ ,NAR,saccharin clearance time and EOS in nasal secretions were significantly lower than those before treatment in two groups,and the levels in observation group were significantly lower than those in control group （ P 〈0 05） ;levels of T lymphocyte subset CD3 ＋ and CD4 ＋ were significantly higher than those before treatment in two groups,and the levels in observation group were significantly higher than those in control group （ P 〈 0 05） . The total effective rate inobservation group （ 91. 89% ）was significantly higher than 74. 67% in control group 1 year after treatment. Conclusion Fluticasone Propionate Nasal Spray combined with electroacupuncture in treatment of patients with perennial allergic rhinitis canachieve better effect than that by Fluticasone Propionate Nasal Spray alone,and it is good for improving patient＇ s condition.
