血小板抗体检测及交叉配型对血小板输注无效的临床意义

Analysis of platelet antibody detection and cross-matching used in treating platelet transfusion refractoriness

目的探讨血小板抗体检测及交叉配型对血小板输注无效的临床意义。方法选择2016年4—12月期间接受血小板输注治疗的患者42例作为研究对象,将检测结果阳性患者21例划入A组,阴性患者21例划入B组。A组患者中接受交叉配型患者10例划入C组,未接受交叉配型患者11例划入D组,比较AB组的输注成功率和CD组的24 h CCI水平、实验时间和费用。计量资料比较采用t检验,计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果 A组输注成功率61.9%低于B组(90.5%,P<0.05)。C组平均输血后24 h CCI水平[(8.5±0.8)×109/L]高于D组[(2.3±0.5)×109/L,P<0.05)C组实验时间、费用[(60.3±2.6)min、(713.5±50.9)元/次]与D组[(59.2±2.4 min)、(715.7±52.3)元/次]相比,差异无统计学意义(均P>0.05)。结论血小板抗体与血小板输注无效密切相关,交叉配型能够提高患者24 h CCI水平。

Objective To observe the effect of platelet antibody detection and cross-matching used in treating platelet transfusion refractoriness Methods A total of 42 patients received platelet transfusion therapy were collected from April to August 2016. Subjects were divided into four groups： A （21 cases, antibody-positive）, B （21 cases, antibody-negative）, C （10 cases, antibody- positive, platelet cross-matching）, D （11 cases, antibody-positive, whithout platelet cross-matching）. The success rate of infusion in group AB and the 24 h CCI level, time and cost in grouop CD were compared. Measurement data uses t test and count data uses X^2 test.P〈0.05 is considered statistically significant. Results In＇ group A, the success rate of platelet transfusion （61.9%） was significantly lower than group B （90.5%, P〈0.05）.In group C, the 24hCCI level （8.5±0.8） 10^9/L） was significantly higher than group D（2.3±0.5） 10^9/L, P〈0.05）. The test time and the experiment cost in group C [（60.3±2.6） min, （713.5±50.9） yuan / time] were not significantly different from group D [（59.2±2.4） min, （715.7±52.3） yuan / time all,P〉0.05]. Conclusions Rate of platelet transfusion refractoriness is closely related to platelet antibody. Cross-matching can improve the level of 24hCCI in patients.
