摘要
目的研究儿童非霍奇金淋巴瘤(NHL)组织中缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)的表达及其意义。方法采用免疫组织化学S-P法检测HIF-1α、VEGF在40例(低度危险组19例、高度危险组21例)儿童NHL淋巴结组织和30例对照组织中的表达情况,统计分析HIF-1α、VEGF的表达与儿童NHL临床病理因素的关系;HIF-1α、VEGF联合高表达与病理分期的关系;HIF-1α、VEGF二者之间Spearman等级相关分析及儿童NHL中高危险的多因素Logistic回归分析。结果 HIF-1α在对照组、低度危险组和高度危险组的阳性表达率分别为0.00%(0/30)、57.89%(11/19)及85.71%(18/21);VEGF在对照组、低度危险组及中高度危险组的阳性表达率分别为0.00%(0/30)、63.16%(12/19)和80.95%(17/21),三组比较差异有统计学意义(P<0.05);二者的阳性表达及联合高表达与儿童NHL的临床病理分期密切相关(P<0.01);Spearman等级相关分析发现,二者的表达呈正相关(r=0.658,P<0.05);多因素Logistic回归分析发现,HIF-1α、VEGF与儿童NHL高度风险明显相关(P<0.01)。结论 HIF-1α、VEGF与儿童NHL发生、发展相关,特别在中高风险期中发挥重要作用,二者的检测在儿童NHL的临床诊断及治疗中具有重要指导意义。
Objective To study the expression and significance of HIF-1α and VEGF in children with non Hochkin's lymphoma(NHL). Methods The immunohistochemistry S-P was used to detect the expression of HIF-1 and VEGF in 40 cases of children's NHL lymph node tissue(low risk group with 19 cases, high risk group with 21 cases) and 30 cases of control tissues. To analyze the relationship between the expression of HIF-1α, VEGF and clinical pathological factors in children with NHL; the relationship between the high expression of HIF-1α, VEGF and pathological staging; the Spearman rank correlation was used to analyze the relationship between HIF-1α and VEGF; the multivariate Logistic regression was used to analyze the middle and high risk in children with NHL. Results The positive rates of HIF-1α in the control group,low risk group and high risk group were 0.00%(0/30), 57.89%(11/19) and 85.71%(18/21) respectively; while the positive rates of VEGF in the control group, low risk group and high risk group were 0.00%(0/30), 63.16%(12/19) and 80.95%(17/21) respectively, the differences among the three groups were statistically significant(P<0.05). The positive expression and joint high expression of the HIF-1α and VEGF were closely related to the clinical pathological staging of NHL in children(P<0.01). The Spearman rank correlation analysis showed that the expression of HIF-1α and VEGF was positively correlated(r=0.658, P<0.05). The multivariate Logistic regression analysis revealed that HIF-1α and VEGF were significantly associated with the high risk of NHL in children(P<0.01). Conclusion HIF-1α and VEGF are related to the occurrence and development of NHL in children, they play an important role in the medium and high risk period particularly, the detection of HIF-1α and VEGF has important guiding significance in the clinical diagnosis and treatment of NHL in children.
