全麻复合硬膜外麻醉在腹腔镜下子宫肌瘤切除术中的应用研究

Application of general anesthesia combined with epidural anesthesia in laparoscopic myomectomy

目的探讨全麻复合硬膜外麻醉在腹腔镜下子宫肌瘤切除术中的应用效果。方法选择2015年3月至2017年1月在本院行腹腔镜下子宫肌瘤切除术治疗的72例患者,随机分为对照组和观察组,各36例。对照组实施全身麻醉,观察组采用全麻复合硬膜外麻醉,观察两组患者的手术时间和术后苏醒情况。结果观察组手术时间为(78.96±10.23)min、术后清醒时间为(10.23±3.01)min、术后拔管时间为(10.58±5.27)min、自主呼吸恢复时间为(6.23±2.01)min、术后疼痛评分为(4.23±3.14)分,均显著优于对照组的(124.56±23.14)min、(21.48±8.57)min、(22.36±8.56)min、(15.44±4.58)min、(6.56±2.56)分(P<0.05)。结论全麻复合硬膜外麻醉在腹腔镜下子宫肌瘤切除术中可达到较好的麻醉效果,值得推广借鉴。

Objective To explore the effect of general anesthesia combined with epidural anesthesia in laparoscopic myomectomy. Methods A total of 72 patients underwent laparoscopic myomectomy in our hospital from March 2015 to January 2017 were randomly divided into control group and observation group, with 36 cases in each group. The control group was given general anesthesia, the observation group was treated with general anesthesia combined with epidural anesthesia. Results The operation time of the observation group was（78.96 ±10.23） min, postoperative awake time was（10.23 ±3.01） min, postoperative extubation time was（10.58 ±5.27） min, the recovery time of spontaneous breathing was（6.23 ±2.01） min, postoperative pain score was（4.23 ±3.14）, which were significantly better than（124.56 ±23.14） min,（21.48±8.57） min,（22.36±8.56） min,（15.44±4.58） min,（6.56±2.56） of the control group（P〈0.05）. Conclusion General anesthesia combined with epidural anesthesia in laparoscopic myomectomy can achieve good effect of anesthesia, which is worthy of promotion.
