腹腔镜手术与开腹手术治疗妊娠期急性阑尾炎的疗效观察

Curative effect observation of laparoscopic surgery and open surgery in the treatment of gestational acute appendicitis

目的观察腹腔镜手术与开腹手术在妊娠期急性阑尾炎中的临床疗效。方法收集我院2011年3月至2016年8月收治的妊娠期急性阑尾炎的患者72例,按照手术方式的不同分为观察组和对照组,每组36例,对照组给予开腹手术,观察组给予腹腔镜手术,观察两组手术方法的临床效果。结果观察组患者手术时间为(89.21±12.43)min,短于对照组的(130.51±20.62)min,观察组术中出血量为(10.93±2.34)mL,少于对照组的(21.33±4.51)mL,观察组的切口长度为(2.47±0.25)cm,短于对照组的(7.42±1.63)cm,观察组术后排气时间为(1.53±0.62)d,短于对照组术的术后排气时间(2.25±1.13)d,观察组患者的住院时间为(3.42±1.24)d,短于对照组的(6.82±1.43)d,两组比较差异均具有统计学意义(P<0.05);观察组的切口感染例数0例,对照组的切口感染例数3例,差异不显著(P>0.05)。结论腹腔镜手术较开腹手术临床效果显著,手术时间短、创伤小、术后恢复快,术中出血量少,缩短了患者的住院时间,提高了治疗效果,是妊娠期急性阑尾炎的首选治疗方法。

Objective To observe the clinical effect of laparoscopic surgery and open surgery in the treatment of appendicitis acute appendicitis. Methods A total of 72 cases of patients with gestational acute appendicitis in our hospital from March 2011 to August 2016 were selected, which were divided into observation group and control group according to the different ways of operation, with 36 cases in each group. The control group was given open surgery while the observation group was given laparoscopic surgery, the clinical effect of the two groups was observed. Results The operation time of the observation group was（89.21 ±12.43） min, which was shorter than（130.51 ±20.62） min of the control group; the amount of bleeding in the observation group was（10.93±2.34） mL, which was less than（21.33±4.51） mL of the control group during operation; the length of incision in the observation group was（2.47 ±0.25） cm, which was shorter than（7.42±1.63） cm of the control group; the postoperative exhaust time of the observation group was（1.53 ±0.62） d, which was shorter than（2.25 ±1.13） d of the control group; the hospitalization time of the observation group was（3.42 ±1.24） d, which was shorter than（6.82 ±1.43） d of the control group, the differences were all statistically significant between the two groups（P 0.05）. There were 0 cases of incision infection in the observation group and 3 cases in the control group, the difference was not significant（ P 0.05）. Conclusion Compared with open surgery, the laparoscopic surgery has significant clinical effect, with shorter operation time, less trauma, faster postoperative recovery, less bleeding, and shorter hospitalization time. Laparoscopic surgery improves the curative effect and it is the preferred treatment method for gestational acute appendicitis.
