闭合复位和切开复位髓内钉内固定治疗股骨干粉碎性骨折的临床疗效分析

Clinical efficiency analysis of closed reduction and open reduction intramedullary nail fixation in treatment of femoral shaft comminuted fracture

目的研究股骨干粉碎性骨折患者采用闭合复位和切开复位髓内钉内固定的治疗效果。方法选取94例股骨干粉碎性骨折患者作为研究对象,按入院顺序分为两组。其中47例患者采用闭合复位髓内钉内固定治疗,作为闭合组;另47例患者采用切开复位髓内钉内固定治疗,作为切开组。观察两组患者手术情况、治疗效果及并发症情况。结果两组患者手术时间无差异(P>0.05);而闭合组出血量、骨折愈合时间及HSS评分均优于切开组(P<0.05);闭合组、切开组治疗优良率分别为91.49%、72.34%,闭合组明显较高(P<0.05);闭合组、切开组并发症总发生率分别为4.26%、17.02%,闭合组明显较低(P<0.05)。结论闭合复位髓内钉内固定治疗股骨干粉碎性骨折效果更佳,术后恢复更快,并发症少,安全性高,值得临床应用及推广。

Objective To study the clinical efficiency of closed reduction and open reduction intramedullary nail fixation in treatment of femoral shaft comminuted fracture. Methods Ninety-four cases of patients with femoral shaft comminuted fracture were selected as the research objects and divided into two groups according to the order of admission. Among them, 47 cases of patients treated with closed reduction ntramedullary nail fixation were served as closed group; the other47 cases of patients treated with open reduction intramedullary nail fixation were served as open group. The surgery situation, treatment efficiency and complications were compared in the two groups. Results There was no difference between the operation duration of the two groups（P〈0.05）. The amount of bleeding, fracture healing time and HSS scores of the closed group were better than those of the open group（P〈0.05）. The excellence and good rates of the closed group and the open group were 91.49% and 72.34% respectively, which of the close group was significantly higher（P〈0.05）. The total incidence of complications in the closed group and the open group were 4.26% and 17.02% respectively, which of the close group was significantly lower（P 0.05）. Conclusion Closed reduction intramedullary nail fixation has better effect in treating femoral shaft comminuted fracture with faster postoperative recovery, fewer complications, and higher safety, which is worthy of clinical application and promotion.
