摘要 目的研究体质量指数(BMI)对哮喘儿童肺脏功能与治疗疗效的影响。方法选取2014年10月至2016年11月该院就诊的哮喘患儿148例,根据患儿的BMI水平将患儿分为对照组(47例)、超重组(52例)、肥胖组(49例)。3组患儿均给予丙酸倍氯米松气雾剂治疗,每次50μg,3次/天,治疗3个月,观察3组患儿治疗前后肺活量(FVC)、用力肺活量占预计值百分比(FVC%)、第1秒用力呼气容积(FEV1)、呼吸峰流速(PEF)、用力呼气50%流速(PEF50)及其改善程度,以及患儿哮喘控制情况。结果治疗前后患儿BMI与FVC、FVC%、FEV1呈负相关,与PEF、PEF50无相关性;治疗后患儿BMI与FVC、FVC%、FEV1的改善情况呈负相关,与PEF、PEF50的改善情况无相关性;患儿BMI水平与哮喘控制有效率呈负相关;3组患儿单位体质量用药量,差异无统计学意义(P>0.05)。结论 BMI水平增高对患儿的肺功能有较大影响,同时也影响治疗效果和疾病控制效率。 ObjectiveTo explore the effect of body mass index(BMI)on lung function and therapeutic efficacy in children with asthma.MethodsOne hundred and forty eight children patients with asthma in the hospital from October 2014 to November 2016 were selected and divided into the control group(47 cases),overweight group(52 cases)and obesity group(49 cases)according to the BMI level.Three groups were given beclometasone aerosol treatment,1.25 μg per time,three times daily for 3 months.The pulmonary function indicators such FVC,FVC%,FEV1,PEF and PEF50,improvement degree and asthma control situation before and after treatment were observed in 3 groups.ResultsThere was a negative correlation between BMI with FVC,FVC% and FEV1 before and after treatment,and there was no correlation between BMI with PEF and PEF50;after treatment,BMI was negatively correlated with the improvement of FVC,FVC% and FEV1,and had no correlation with the improvement of PEF and PEF50;the BMI level was negatively correlated with asthma control effective rate;the unit body mass dose had no statistically significant difference among the 3 groups(P〉0.05).ConclusionThe BMI level increase has a great influence on children's lung function,meanwhile also influences the treatment effect and disease control efficiency.
