西吡氯胺含漱液对心脏直视手术后呼吸机相关性肺炎的防治效果

The Preventive Effect of Cetylpyridinium Chloride Gargle on Ventilator-associated Pneumonia After Open Heart Surgery

目的探讨西吡氯胺对心脏直视手术患者呼吸机相关性肺炎(VAP)的预防效果。方法选择2017年3—6月,我科收治的186例行心脏直视手术患者作为研究对象。随机分为对照组和观察组各93例,对照组进行常规的口腔护理,观察组采用西吡氯胺行围术期口咽部清洁,对比两组患者术后VAP的发生率。结果观察组患者早期VAP发生率低于对照组,差异具有统计学意义(P<0.05),晚期VAP发生率相同。结论心脏直视手术患者使用西吡氯胺行围术期口咽部清洁能有效地减少病原菌在口咽部的繁殖和下移,减少早发型VAP的发生。

Objective To study the preventive effect of cetylpyridinium chloride on ventilator associated pneumonia（VAP） in the patients after open heart surgery. Methods From March to June 2017, 186 patients underwent open heart surgery were studied in Cardiovascular Surgery Department, who were randomly divided into control group and observation group , and there were 93 cases in each group. The patients in control group who underwent routine oral care, and the patients in observation group used cetylpyridinium chloride for oral care in perioperative period. The incidence of VAP in postoperative patients between the control group and the observation group was compared. Results In early stage, the rate of VAP in the observation group was significantly lower than the control group （P 〈 0.05）, but there was no significant difference in the rate of VAP with later period. Condusion Using cetylpyridinium chloride for oral care in the patients who undergo open heart surgery, that can effectively restrain the reproduction of pathogenic bacteria, and reduce the incidernce of early-onset VAP.
