颈动脉粥样硬化的超声特征及其对冠心病病情的预测价值分析

目的分析颈动脉粥样硬化的超声特征及其对冠心病病情的预测价值。方法选取65例冠心病患者作为试验组,选取同期健康体检者65例作为对照组,两组受试者均行颈动脉超声检查,分析内膜中层厚度与颈动脉病变程度相关性,及其对冠心病病情预测价值,并对炎症反应指标和血脂代谢指标进行比较。结果试验组颈动脉IMT明显高于对照组,差异具有统计学意义(P<0.05);两组血浆CCL21、FFA、LDL-C、CCR7、s CD40L比较,对照组低于试验组(P<0.05);两组IL-10、MFG-E8、HDL-C/LDL-C比例比较,对照组高于试验组(P<0.05);试验组IMT不同患者炎症反应指标、血脂代谢指标差异明显,血浆中s CD40L、CCR7、LDL-C、FFA、CCL21含量与IMT厚度成正比,HDL-C/LDL-C、IL-10、MFGE8、HDL-C含量与IMT厚度成反比。结论颈动脉IMT增厚对评定冠心病患者的血脂代谢情况、炎症反应程度及冠脉病变数目具有重要意义,值得临床普及推广。

Objective To analyze the ultrasonographic features of carotid atherosclerosis and its predictive value for coronary heart disease.Methods 65 cases of coronary heart disease patients as a group, select the same physical examination, 65 cases as control group, 2 group, all the subjects to carotid ultrasound examination, with the intima-media thickness of carotid artery lesion degree of judgment, and inflammation index and blood lipid metabolism index were collected and compared. Results The carotid IMT was significantly higher than the control group, and the difference was statistically significant(P<0.05). 2 groups of plasma CCL21, FFA, LDL-C, CCR7, s CD40 L were compared, compared with the control group(P<0.05). 2 groups of IL-10, MFG-E8, HDL-C/LDL-C ratio were compared, compared with the control group(P<0.05). Team of IMT in patients with different inflammation index, blood lipid metabolism index difference obvious, s CD40 L, when the CCR7, LDL-C in plasma, FFA,CCL21 content is proportional to the thickness of IMT, HDL-C/LDL-C, IL-10, MFGE8, HDL-C content is inversely proportional to the thickness of IMT. Conclusion The thickening of the carotid artery IMT is of great significance to the evaluation of blood lipid metabolism, inflammatory response and coronary artery disease in patients with coronary heart disease, and it is worth popularizing in clinic.
