巴林特小组在住培医师规范化培训中的探索与实践

Exploration and practice of Balint group in the standardized training of resident doctors

目的 :探索巴林特小组活动在参与规范化培训的住院医师(住培医师)中开展的可行性和有效性。方法 :2015年11月至2017年4月在同济大学附属同济医院开展了26次巴林特小组活动,先后共有53名住培医师参与,每次活动有8~12人参加,时间约1 h。采用自设问卷对53名住培医师进行调研,评估巴林特小组活动的可行性和有效性。结果 :住培医师对巴林特小组活动安排的满意度较高。45人(84.91%)认为对"感受到理解和支持"非常有帮助,38名(71.70%)认为对"释放工作压力"非常有帮助,37人(69.81%)认为对"学会耐心倾听"和"共情能力提升"非常有帮助。超过50%的参与者表示对"医患沟通能力"、"职业认同感提升"、"自我心理调适"等方面有帮助,42名(79.25%)住培医师表示愿意继续参加巴林特小组活动,主要理由是"提高医患交流能力和职业素养"。结论 :巴林特小组活动对缓解住培医师职业压力、提高医患沟通能力和预防职业倦怠方面起有积极的促进作用,建议在住培医师规范化培训中推广。

Objective： To explore the feasibility and efectiveness of the activities of Balint group in the standardized training of resident doctors. Methods： From November 2015 to April 2017, 26 Balint group activities were carried out in Tongji Hospital of Tongji University, and 53 resident doctors participated in the standardized training . Eight to twelve participants attended the activities for about one hour in each activity. The self-designed questionnaire was used to investigate 53 participants and the feasibility and efectiveness of Balint group activities were evaluated. Results： Resident doctors were more satisfed with the activity arrangement of Bahrain group. The item of “perceiving the understanding and support” was considered very helpful in 45 （84.91%） participants. The item of “releasing occupational pressure” was considered very helpful in 38 （71.10） participants. The items of “learning how to listen to patients and improving their empathy ability” were considered very helpful in 37 （69.81%） participants. The items of “improving their physician-patient communication skills, professional identify and self-psychology adjustment” were considered helpful in more than 50% participants. Resident doctors expressed their willingness to continue toparticipate in Bahrain group activities in the future, and the main reason was to “improve the physician-patient communication ability and professionalism”. Conclusion： Balint group activities play an active role in alleviating the professional pressure of resident doctors, improving doctor-patient communication ability and preventing the job burnout. It is recommended to promote Balint group activities in the standardized training program of resident doctors.
