家庭康复护理对农村脑卒中患者日常生活活动能力的影响

Effect of home-based rehabilitation nursing on the ability of activities of daily living in the rural patients with stroke

目的 :探索基于全科团队模式的家庭康复护理干预对农村脑卒中后遗症期患者日常生活活动能力的影响。方法 :将农村社区84例脑卒中后遗症期患者进行随机分组,干预组42例,其中男性23例,女性19列,平均年龄(69.71±6.61)岁;对照组42例,其中男性21例,女性21列,平均年龄(70.60±6.23)岁。干预组由全科团队中的护士实施家庭康复护理干预,分别于干预后1个月、3个月、6个月和12个月采用Barthel指数(BI指数)评分来评价患者的日常生活活动能力。结果 :两组间在基线时和干预后1个月时的BI指数评分差异无统计学意义(P>0.05),在干预后3个月、6个月和12个月时,干预组的BI指数评分和提高分均明显高于对照组,差异均有统计学意义(P<0.01)。结论 :社区全科团队模式下的家庭康复护理干预能提高农村脑卒中后遗症期患者的日常生活活动能力。

Objective： To explore the effect of home-based rehabilitation nursing intervention based on general practitioner team mode on the ability of activities of daily living in patients with sequelae of stroke in rural areas. Methods： Eighty-two patients with stroke sequelae in the rural community were randomly divided into an intervention group with 42cases, including 23 males and 19 females, with an average age of （69.71 ± 6.61） years and the control group with 42 patients, including 21 males and 21 females, with an average age of （70.60 ± 6.23） years. The intervention group received the home-based rehabilitation nursing intervention by the nurses in the general practitioner team, and the Barthel index （BI index） score was used to evaluate the ability of activities of daily living of the patients after the intervention of 1, 3, 6 and 12 months. Results： There was no signifcant diference in the BI index score between the two groups at baseline and after intervention of 1 month （P 〉 0.05）. After intervention of 3, 6 and 12 months, the BI index scores and the improvement scores of the intervention group were signifcantly higher than those of the control group, and the diferences were statistically signifcant （P 〈 0.01）. Conclusion： Home-based rehabilitation nursing intervention can improve the ability of activities of daily living in patients with stroke sequelae in the rural community under the general practitioner team mode.
