尿道下裂TIP手术后尿瘘与手术年龄的相关性分析

Relative study on age and the incidence of urinary fistula after hypospadias TIP

目的比较某三甲医院各年龄组患儿行TIP尿道成形手术的一般资料,探讨一期尿道下裂TIP手术的最佳年龄。方法回顾性分析2009年至2017年本院收治的阴茎远端型和阴茎中段型尿道下裂一期TIP尿道成形手术患儿临床资料,比较不同年龄患儿手术后尿瘘发生的差异。结果本组共234例,尿瘘发生率12.8%,其中>6岁组尿瘘发生率29.8%,显著高于<1岁组(3.2%)、1~3岁组(4.9%)和3~6岁组(13.5%),差异有统计学意义(P<0.029)。结论尿道下裂TIP手术后尿瘘的发生受患儿手术年龄影响,且随年龄增加而呈上升趋势,选择合适的年龄手术能有效降低术后尿瘘的风险。

Objective Retrospectively analysis of patients with hypospadias TIP,to investigate the impact of age on the incidence of urinary fistula after urethroplasty in a tertiary hospital. Methods Between2009 and 2017,Patients with distal and mid-penile hypospadias underwent the TIP procedure were collected.The differences between the incidence of fistula and the age at surgery were compared. Results A total of 234 children were included. Fistula rate is 12. 8%. The incidence of urinary fistula in 6 year group was 29. 8 %,which was significant higher than that of 1 year old group（ 3. 2 %）,1 ～ 3 years old group（ 4. 9 %） and3 ～ 6 years old group（ 13. 5 %）. They have significantly statistical differences（ P〈0. 029）.Conclusion The incidence of urinary fistula after TIP urethroplasty were influenced by the age at surgery,which has a increase trends with ages. Choosing suitable operation age could significantly reduce the risk of urinary fistula after surgery.
