临床药师参与3例肿瘤患者化疗后骨髓抑制治疗的药学服务实践

Participation of clinical pharmacists in the treatment of 3 cancer patients suffering myelosuppression after the chemotherapy

目的本文拟通过具体的病例实践,来思考临床药师的工作模式及思维方式,为其他药师提供参考。方法临床药师参与了3例肿瘤患者化疗后骨髓抑制治疗的全过程,重点监护了粒细胞集落刺激因子的给药途径、给药时机及给药疗程的合理性。结果临床药师结合患者的自身情况及所使用药物的特点,协助医师为患者制定了更加合理的治疗方案。结论临床药师参与临床实践,不能照搬说明书和指南,从专业角度发挥自身优势,并针对不同个体优化给药方案,携手医师共同提高患者用药的安全性,有效性和经济性。

Objective To explore the work pattern and thinking mode of clinical pharmacists by analyzing specific cases, and to provide reference for other pharmacists. Methods Clinical pharmacists participated in the whole treatment process of 3 patients with myelosuppression after the chemotherapy, focusing on the rationality of delivery route, delivery time, and treatment course of granulocyte colony stimulating factors. Results Clinical pharmacists assisted doctors in developing more reasonable treatment plans for patients by taking into account both the patients＇ physical condition and the characteristics of the drugs. Conclusion In clinical practice, clinical pharmacists should not simply copy or follow the instructions or guidelines, but should consider individual differences, to improve the safety, effectiveness and economy efficiency of the drug use.
