基层医院神经外科危重颅脑外伤救治临床分析

Clinical Analysis of Emergency of Critical Cerebral Injury in the Department of Neurosurgery in the Primary Hospital

目的研究基层医院神经外科危重颅脑外伤的救治要点。方法于2016年6月—2017年6月在该院神经外科收治的危重颅脑外伤患者中选出52例为研究对象,回顾性分析患者的抢救措施以及救治效果。结果 6例患者死亡,46例患者存活,存活率为88.46%;46例存活患者治疗14 d时的NIHSS评分(19.29±1.58)分、3个月时的NIHSS评分(14.30±1.02)分明显低于治疗前(28.18±2.45)分(P<0.05);46例存活患者治疗14 d时的GCS评分(9.89±1.83)、3个月时(11.07±2.37)分明显低于治疗前(5.75±1.58)分(P<0.05)。结论基层医院神经外科在救治危重颅脑外伤患者可采取综合急诊救治法,综合考虑患者的各方面情况,快速抢救,预防并发症发生,改善预后。

Objective To research the emergency key points of critical cerebral injury in the department of neurosurgery in the primary hospital. Methods 52 cases of patients with critical cerebral injury admitted and treated in our hospital from June 2016 to June 2017 were selected as the research objects and the rescue measures and emergency effect of patients were retrospectively analyzed. Results 6 cases died and 46 cases survived, and the survival rate was88.46%, and the NIHSS scores at 14 d and 3-month treatment of 46 cases were obviously lower than those before treatment[（19.29±1.58）points,（14.30±1.02）points vs（28.18±2.45）points]（P〈0.05）, and the GCS score of 46 cases at 14 d, 3-month treatment were obviously lower than that before treatment [（9.89 ±1.83）points,（11.07 ±2.37）points vs（5.75 ±1.58）points]（P〈0.05）. Conclusion We should consider all aspects when using comprehensive emergency method to treat the patients with critical cerebral injury in the department of neurology in the primary hospital thus preventing the occurrence of complications and improving the prognosis.
