卡培他滨单药治疗老年晚期乳腺癌的临床分析

Clinical Analysis of Single Drug of Capecitabine in Treatment of Elderly Advanced Breast Cancer

目的观察单药卡培他滨治疗老年晚期乳腺癌的临床疗效及不良反应。方法随机选取2012年5月—2017年5月吉林医药学院附属医院的36例老年晚期乳腺癌患者应用单药卡培他滨化疗回顾性分析,卡培他滨2 000 mg/m^2,分2次口服,第1~14天,21 d为1个周期,每两周期评价疾病疗效并观察其主要不良反应。结果36例老年乳腺癌患者的中位化疗周期数为4个,其中临床完全缓解(CR)为2例,临床部分缓解(PR)为9例,疾病稳定(SD)为15例,疾病进展(PD)为10例,总有效率(OR)为30.6%(11/36),疾病控制率(DCR)为72.2%(26/36)。治疗有效患者的中位疾病进展时间(TTP)为6个月,中位总生存时间(OS)为9.5个月。主要的不良反应包括骨髓抑制、消化道反应及口腔黏膜炎等,大多数反应为Ⅰ~Ⅱ度,Ⅲ~Ⅳ度不良反应主要为2例(12.5%)粒细胞减少和1例(6.7%)手足综合征。结论卡培他滨单药治疗老年晚期乳腺癌可有效地控制疾病进展且不良反应可以耐受。

Objective To observe the clinical curative effect and adverse reactions of single drug of capecitabine in treatment of elderly advanced breast cancer. Methods 36 cases of senile patients with advanced？breast cancer admitted and treated in our hospital from May 2012 to May 2017 were randomly selected, and were given the 2000 mg/m^2 capecitabine, oral administration twice from 1 d to 14 d with a cycle of 21 days, and the disease curative effect was evaluated every two cycles and the major adverse reactions were observed. Results The median chemotherapy cycle number of 36 cases was 4, and the clinical complete relief was in 2 cases, clinical partial relief was in 9 cases, disease stability was in 15 cases, disease progress was in 10 cases, and the total effective rate was 30.6%（11/36）, and the disease control rate was 72.2%（26/36）, and the median disease progress time was 6 months, and the total median survival time was 9.5 months, and the major adverse reactions included the myelosuppression, gastrointestinal reactions and oral mucositis, most reactions was Ⅰ ~ Ⅱ degree, and the adverse reactions of Ⅲ ~ Ⅳ degrees were in 3 cases, including 2 cases with granulocytopenia（12.5%） and 1 case with hand-foot syndrome（6.7%）. Conclusion The single drug of capecitabine in treatment of elderly advanced breast cancer can effectively control the disease progress and adverse reactions.
