综合康复训练联合咽部冰刺激治疗老年急性脑卒中后吞咽困难的临床疗效

Effect of pharyngeal ice stimulation on dysphagia after acute stroke in elderly patients

目的分析综合康复训练联合咽部冰刺激治疗老年急性脑卒中后吞咽困难患者的临床效果。方法急性脑卒中后出现吞咽困难的老年患者96例随机分为两组,各48例。对照组接受药物治疗及进食训练、口腔肌肉锻炼等综合康复训练。观察组在对照组治疗的基础上联合咽部冰刺激。比较两组治疗有效率和吸入性肺炎发生率,同时观察治疗前后两组Barthel指数和生活质量量表评分变化。结果观察组治疗总有效率显著高于对照组(P<0.05),吸入性肺炎发生率显著低于对照组(P<0.05),Barthel指数和生活质量量表评分均显著高于对照组(P<0.05)。结论综合康复训练联合咽部冰刺激应用于急性脑卒中后吞咽困难,能够显著提高治疗效果,降低吸入性肺炎的发生率,提高患者的生活质量。

Objective To investigate the effect of pharyngeal ice stimulation on dysphagia after acute stroke in ehlerly patients. Methods 96 patients with dysphagia after acute stroke in hospital from February 2015 to January 2016 were randomly divided into two groups, 48 cases in each group. Control group received nursing intervention, oral muscle exercise, drug therapy. Observation group received control group treatment based on the combination of pharyngeal ice stimulation. Efficiency and incidence of aspiration pneumonia were com- pared, and the changes of Barthel index and quality of life were observed in the two groups before and after treatment. Results The total ef- fective rate in observation group was significantly higher than that of control group （ P〈O. 05 ） ; The incidence of aspiration pneumonia in obser- vation group was significantly lower than that of control group （P〈0.05） ;After treatment, the Barthel index and quality of＂ life in observation group were significantly higher than those of control group（ P〈O. 05 ）. Conclusions Pharyngeal ice stimulation has very high application val- ue in the treatment of elderly patients with acute stroke patients with dysphagia ＇after, which can significantly improve therapeutic effect, re- duce inhalation pneumonia incidence, improve the quality of life of patients.
