少腹逐瘀胶囊治疗寒凝血瘀型子宫腺肌病临床研究

Clinical Study of Shaofu Zhuyu Capsules for Adenomyosis of Congealing Cold and Blood Stasis Type

目的:观察少腹逐瘀胶囊治疗寒凝血瘀型子宫腺肌病(AM)的临床疗效,及对患者血清CA125、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平的影响。方法:选取300例寒凝血瘀型AM患者,按随机数字表法分为观察组与对照组各150例。2组均在患者宫底放置曼月乐环,观察组加服少腹逐瘀胶囊,对照组加服孕三烯酮胶囊。观察2组治疗后中医证候积分、痛经评分、子宫体积及血清CA125、IL-8、TNF-α水平的变化,评估临床疗效。结果:治疗后,2组中医证候积分及痛经评分均较治疗前降低(P<0.05),观察组的中医证候积分及痛经评分均较对照组下降更明显(P<0.05)。治疗后,2组子宫体积均较治疗前缩小(P<0.05),观察组的子宫体积比对照组缩小更明显(P<0.05)。治疗后,2组血清CA125、IL-8及TNF-α水平均较治疗前降低(P<0.05),观察组的血清CA125、IL-8及TNF-α水平均低于对照组(P<0.05)。观察组治疗总有效率92.00%,高于对照组的75.33%(P<0.05)。结论:少腹逐瘀胶囊治疗寒凝血瘀型AM,可改善患者的痛经及其他临床症状,促进子宫恢复,并能改善患者血清CA125、IL-8和TNF-α水平,临床疗效显著。

Objective. To observe the clinical effect of Shaofu Zhuyu capsules for adenomyosis （AM） of congealing cold and blood stasis type and the effect on levels of serum CA125, interleukin-8 （IL-8） and tumor necrosis factor-α （TNF-α）. Methods- Selected 300 cases of AM patients of congealing cold and blood stasis type, and divided them into the observation group and the control group randomly, 150 cases in each group, Both groups were placed Mirena in uterine. The observation group additionally received Shaofu Zhuyu capsules, and the control group additionally received gestrinone capsules.After treatment, observed Chinese medicine symptom score, dysmenorrhea score, uterine volume and changes of levels of CA125, IL-8 and TNF-cc. Assessed the clinical effect. Results： After treatment, Chinese medicine symptom scores and dysmenorrhea scores in both groups were decreased when compared with those before treatment（P〈 0.05）. The two scores in the treatment group were reduced more significantly than those in the control group（P 〈 0.05）. After treatment, uterine volume in both groups was reduced when compared with that before treatment（P〈 0.05）. Uterine volume in the treatment group was reduced more significantly than that in the control group（P〈 0.05）. After treatment, levels of serum CA125, IL-8 and TNF-α in both groups were declined when compared with those before treatment（P 〈 0.05）. Levels of CA125, IL-8 and TNF-α in the treatment group were lower than those in the control group （P 〈0.05）. The total effective rate was 92.00% in the observation group, being higher than 75.33% in the control group（P〈 0.05）. Conclusion= Shaofu Zhuyu capsules for AM of congealing cold and blood stasis type can effectively improve dysmenorrhea and other clinical symptoms, promote recovery of uterine and improve levels of serum CA125, IL-8 and TNF-α, which has significant clinical effect.
