星状神经节阻滞在肿瘤晚期患者中的临床应用

The clinical application of stellate ganglion block in the treatment of advanced tumor patients

目的探讨星状神经节阻滞在肿瘤晚期患者疼痛和生活质量改善中的应用价值,并评价其疗效。方法选取癌痛晚期患者160例,按照随机数字表法将患者随机分为对照组与观察组,每组各80例。对照组患者给予塞来昔布胶囊治疗,观察组患者给予星状神经节阻滞联合塞来昔布胶囊治疗,根据患者的自身情况及治疗需要调节药物用量。结果两组患者治疗后的总有效率比较,差异无统计学意义(P﹥0.05)。经治疗后,塞来昔布胶囊用量较首次用量增加的患者对照组有48例,观察组有8例,两组患者塞来昔布胶囊用量较首次用量增加率比较,差异有统计学意义(P﹤0.05);观察组患者的失眠、食欲减退、乏力、疼痛、对治疗的态度改善情况均优于对照组(P﹤0.05)。两组患者的总不良反应发生率比较,差异无统计学意义(P﹥0.05)。结论采用星状神经节阻滞联合塞来昔布胶囊治疗,可降低药物浓度、缓解患者因肿瘤所引起的疼痛、食欲减退和疲乏症状,提高患者的依从性。

Objective To investigate the value of stellate ganglion block in improving the pain and quality of life in patients with advanced cancer. Method A total of 160 cases of advanced cancer patients were randomized as control group or study group, with 80 patients in each. Patients in the control group was given celecoxib capsules, while those in study group were given stellate ganglion block combined with celecoxib treatment, and the dose was modified according to the individual conditions. Result There was no significant difference in regard of the overall response rate between the two groups（P〉0.05）. After treatment, there were 48 and 8 cases of increased use of celecoxib compared with initial dose in control group and study group, respectively, and the difference was significantly different（P〈0.05）; a higher degree of improvement in respect of insomnia, loss of appetite, fatigue, pain, and attitude towards the therapy was observed in study group compared with the control group（P〈0.05）. The overall incidence of adverse reactions in the two groups were of no significant difference（P〉0.05）. Conclusion The combined therapy of stellate ganglion block and celecoxib may reduce the concentration of medicine, relieve cancer pain, and can also improve symptoms such as decreased appetite and fatigue, thus improving the compliance for patients.
