腹腔镜辅助经脐入路治疗小儿卵巢囊肿蒂扭转6例报告

Laparoscopic-assisted Transumbilical Resection for Ovarian Cyst Pedicle Torsion in Children:Report of 6 Cases

目的探讨腹腔镜辅助经脐入路治疗小儿卵巢囊肿蒂扭转的临床价值。方法 2012年3月~2015年3月对6例小儿卵巢囊肿蒂扭转行急诊腹腔镜辅助经脐入路探查、卵巢囊肿切除术,术中行囊液穿刺减压后,经脐扩大切口将囊肿提出腹腔外行囊肿剥除术。结果 6例均顺利完成手术,成功保留患侧卵巢,手术时间45~90 min,(50±22)min;术后住院3~5 d,平均4 d。6例随访6~12个月,平均10个月,腹部B超检查无复发,切口瘢痕小,脐部外观满意。结论腹腔镜辅助经脐入路手术具有创伤小、恢复快、操作简单、易推广等优点,对于腹腔镜手术操作困难的患者是一种较好选择。

Objective To investigate the clinical value of laparoscopic-assisted transumbilical resection for ovarian cyst pedicle torsion in children. Methods A total of 6 cases of ovarian cyst pedicle torsion between March 2012 and March 2015 were retrospectively analyzed. All the patients underwent emergent laparoscopic-assisted exploration and resection of ovarian cyst. After the cyst puncture decompression,the umbilical incision was enlarged to pull the cyst outside the abdominal cavity for cystectomy.Results All the 6 cases of laparoscopic-assisted resection were successfully performed with ovarian reservation. The operation time was 45-90 min（50 ± 22 min） and the patients were discharged from hospital within 3-5 days（mean,4 days）. All the cases were followed up for 6-12 months（mean,10 months）. B-ultrasonic examination showed no recurrence and satisfactory cosmetic results.Conclusion Laparoscopic-assisted transumbilical excision of ovarian cyst has advantages of minimal invasion,rapid recovery,simple performance,and easy promotion,being a good choice for the treatment of complex ovarian cysts in children.
