保肺康汤对COPD合并肺间质纤维化患者血气、肺功能指标与胸部影像学的影响

Effect of Modified Xiao Qinglong Decoction on Serum Inflammatory Factors Levels and Cardiopulmonary Function for Patients with Pulmonary Heart Disease

目的:观察保肺康汤对慢性阻塞性肺疾病(COPD)合并肺间质纤维化患者血气、肺功能指标与胸部影像学的影响。方法:选取我院2013年7月~2016年7月68例COPD合并肺间质纤维化患者为研究对象,将纳入患者抽签随机分为中药组与常规组,每组34例。常规组给予常规治疗,中药组在常规组基础上给予保肺康汤,比较两组临床疗效、血气分析指标、肺功能指标、胸部影像学评分。结果:中医组的治疗总有效率、氧分压(PaO_2)、肺弥散功能(DLCO)分别为70.6%、(63.71±7.36)mm Hg、(58.92±12.17)显著高于常规组的47.1%、(56.24±7.22)%、(52.11±14.38)%(P<0.05)。中医组治疗后胸部影像学评分为(17.68±6.42)分与常规组的(19.84±6.91)分比较差异无统计学意义(P>0.05)。结论:保肺康汤治疗COPD合并肺间质纤维化可以改善肺通气与换气状态,延缓肺间质纤维化进程。

Objective： To study the effect of Modified Xiao Qinglong Decoction on levels of serum inflammatory factors andcardiopulmonary function in patients with pulmonary heart disease. Methods ： A total of 80patients with pulmonary heart diseaseadmitted to our hospital during August 2014to August 2016were selected as subjects. The patients were randomly divided into tra-ditional Chinese medicine （TCM） group and routine group, 40cases in each group. The routine group were given routine treat-ment of western medicine, on the basis, the TCM group were treated with Modified Xiaoqinglong Decoction. The clinical effica-cy, levels of inflammatory factors, blood gas indicators and cardiopulmonary function indexes were compared between the twogroups. Results： The total effective rate of the TCM group was significantly higher in than the routine group （90. 0% vs72. 5% ） （P〈O. 05）. There were significant differences in interleukin-8 （IL-8） , partial pressure of blood oxygen （ PaO2 ） ,cardiac output （CO） and forced vital capacity （FVC） between the TCM group and the routine group [ （306. 49±130. 47） ng/L,（82.37±16.47） mmHg, （4.02±0.57） L/rain, （3.35±0.84） Lvs （424.18±116.52） ng/L, （72.59±16.65） mmHg,（3.49±0. 63） L/min, （2.87±0. 58） L] （P〈0. 05）. Conclusion： Modified Xiaoqinglong Decoction can reduce the airway in-flammation, relieve the elinieal symptoms of dyspnea, and promote the recovery of cardiopuhnonary function in patients with pul-monary heart disease.
