抗体检测技术在肿瘤化疗患者血小板输注中的应用

摘要 目的探讨抗体检测技术应用于肿瘤化疗患者血小板输注的临床效果。方法选取我院2012年10月至2016年10月收治的95例肿瘤化疗患者,均进行血小板抗体检测,并将检测结果呈阳性的患者分为研究组和对照组。研究组在血小板ABO同型条件下依据血小板抗体类型进行交叉配型后输注,对照组未经血小板配型直接输注同型血小板。比较两组患者的临床治疗情况。结果 95例肿瘤化疗患者经血小板抗体检测,阳性率为44.21%(42例),阳性者均有>1次输血史,随着输注次数增多,患者的血小板抗体阳性率不断提高;研究组输注血小板1 h、24 h后的CCI均明显高于对照组,其输注后1 h PPR<30%和24 h PPR<20%比例均明显低于对照组,差异有统计学意义(P<0.01)。结论肿瘤化疗患者进行血小板抗体检测与交叉配型后输注,能够有效提高血小板输注有效率,临床应用价值较高。 Objective To explore the clinical effect of antibody detection technique in platelet transfusion of patients with tumor chemotherapy. Methods 95 cases of patients treated with cancer chemotherapy admitted to our hospital from October 2012 to October 2016 were selected and given platelet antibody detection. Then the patients with positive detection result were divided into study group and control group. Under the condition ofplatelet ABO isoform, the study group was transfused after cross-matching according to platelet antibody type. The control group received the isotype platelet transfusion directly without platelet cross-matching. The clinical treatment of patients was compared. Results After platelet antibody detection, the positive rate was 44.21% (42 cases) among 95 cases of patients with cancer chemotherapy. All positive patients had 〉1 times blood transfusion history. With the increase in transfusion times, the positive rate ofplatelet antibody continued to increase. The CCI at 1 h and 24 h after platelet transfusion of study group were significantly higher than those of control group, the rates of 1 h PPR〈30% and 24 h PPR〈20% after transfusion were significantly lower than those of control group, with statistical differences (P 〈0.01). Conclusions Platelet antibody detection and transfusion after cross-matching in patients with tumor chemotherapy can effectively improve the effective rate of platelet transfusion with higher clinical application value.
