雷火灸联合99锝-亚甲基二磷酸盐对强直性脊柱炎肾虚督寒证患者关节功能及炎症指数的影响

Influence of Thunder-Fire Moxibustion Combined with ^(99)Tc-MDP on the Joint Function and Inflammatory Indicators in Patients with Ankylosing Spondylitis of Kidney Deficiency and Governor Vessel Cold Syndrome

目的:观察雷火灸联合双磷酸盐对强直性脊柱炎肾虚督寒证患者病情活动的影响及对患者功能活动的改善情况,观察其对炎性因子的影响。方法:选择本院确诊为强直性脊柱炎肾虚督寒证患者91例,随机分成观察组31例,对照Ⅰ组30例,对照Ⅱ组30例。观察组用雷火灸联合99锝-亚甲基二磷酸盐(99 Tc-methylene diphosphate,99 Tc-MDP,商品名云克)治疗,对照Ⅰ组使用普通艾灸联合99锝-亚甲基二磷酸盐治疗,对照Ⅱ组单用99锝-亚甲基二磷酸盐治疗。3组均以15 d为1个疗程,连续两个疗程后评价各项指标。观察血沉、C反应蛋白、中医证候评分、脊柱疼痛(visual analogue scale,VAS评分)、功能指数(bath ankylosing spondylitis function inden,BASFI)、疾病活动性指数(actioities disease index,BASDI)等。结果:对照I组有效率为76.67%,对照Ⅱ组有效率为53.33%,观察组有效率为96.77%,观察组有效率高于对照Ⅰ组和对照Ⅱ组(P<0.05)。3组患者脊柱疼痛(VAS评分)、功能指数(BASFI)、脊柱炎症(BASDI)与治疗前比较,差异有统计学意义(P<0.01)。结论:雷火灸联合99锝-亚甲基二磷酸盐治疗强直性脊柱炎肾虚督寒证,对改善强直性脊柱炎临床症状有明显疗效。

Objective： To observe the influence of thunder-fire moxibustion combined with Bisphosphonates on the disease condition of patients with ankylosing spondylitis（ AS） of kidney deficiency and governor vessel cold syndrome. Methods： Totally91 patients with AS were randomly divided into control groupⅠ（ n = 30,treated with regular moxibustion ＋ Yunke ivgtt qd）,control groupⅡ（ n = 30 only treated with Yunke ivgtt qd）,and observation group（ n = 31,treated with thunder fire moxibustion ＋Yunke ivgtt qd）. Fifteen days as a treatment course for three groups. After two courses,changes of erythrocyte sedimentation rate（ ESR）,C-Reactive protein（ CRP）,TCM symptom scale,spinal pain degree（ VAS）,bath AS functional index（ BASFI）,bath AS disease activity index（ BASDAI） were observed. Results： The effective rate was 76. 67% in control group I,53. 33% in control groupⅡ and 96. 77% in observation group; observation group were superior to the other two groups（ P〈0. 05）. Furthermore,the VAS,BASFI and BASDI in three groups significantly improved; the difference had statistical significance（ P〈0. 01）. Conclusion： Yunke combined with thunder-fire moxibustion can significantly improve the symptoms in patients with AS of kidney deficiency and governor vessel cold syndrome.
