布地奈德联合孟鲁司特钠对慢性阻塞性肺疾病急性发作患者血清炎性因子和肺功能的影响

Influence of budesonide combined with montelukast sodium on serum inflammatory cytokines and pulmonary function of patients with acute exacerbation of chronic obstructive pulmonary disease

目的探讨布地奈德联合孟鲁司特钠对慢性阻塞性肺疾病(COPD)急性发作患者血清炎性因子和肺功能的影响。方法选取福建省漳州市人民医院2016年1月—2017年1月收治的COPD急性发作患者86例,按照随机数字表法分为对照组(n=43)和观察组(n=43)。对照组患者给予布地奈德治疗,观察组患者在对照组基础上加用孟鲁司特钠治疗,两组患者均持续治疗2周。比较两组患者治疗前后血清炎性因子[肿瘤坏死因子α(TNF-α)、白介素6(IL-6)、白介素8(IL-8)]、肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]。结果治疗前,两组患者TNF-α、IL-6、IL-8水平及FEV1、FVC、FEV1/FVC比较,差异无统计学意义(P>0.05);治疗后,观察组患者TNF-α、IL-6、IL-8水平低于对照组,FEV1、FEV1/FVC高于对照组(P<0.05),两组患者FVC比较,差异无统计学意义(P>0.05)。结论采用布地奈德联合孟鲁司特钠治疗COPD急性发作效果明显,可有效降低患者的炎性反应,提高肺功能,改善预后。

Objective To investigate the influence of budesonide combined with montelukast sodium on serum inflammatory cytokines and pulmonary function of patients with acute exacerbation of chronic obstructive pulmonary disease（ COPD）.Methods A total of 86 cases of acute exacerbations of COPD patients were selected from January 2016 to January 2017 in Zhangzhou People’s Hospital of Fujian Province,which were divided into the control group（ n = 43） and observation group（ n = 43）according to random number table. Control group was given budesonide,observation group was given montelukast sodium on the basis of control group,both groups were treated for 2 weeks. The serum inflammatory factors（ TNF-α,IL-6,IL-8） and pulmonary function index（ FEV1,FVC,FEV1/FVC） before and after treatment were compared between the two groups. Results Before treatment,no statistically significant differences of TNF-α,IL-6,IL-8 levels or FEV1,FVC,FEV1/FVC was found between the two groups（ P 〉 0. 05）; after treatment,the observation group of TNF-α,IL-6,IL-8 levels were lower than control group,FEV1,FEV1/FVC were higher than control group（ P 〈 0. 05）,no significant differences of FVC was found between the two groups（ P 〉 0. 05）. Conclusion Budesonide combined with montelukast sodium have an exactly clinical effect in the treatment of patients with acute exacerbation of COPD,it can reduce inflammation factors of patients,and promote lung function,improve the prognosis.
