锁定加压钢板固定术与交锁髓内钉固定术治疗肱骨干中段骨折的疗效比较研究

Comparative effectiveness research of locking compression plate fixation and interlocking intramedullary nail fixation in treating middle humeral shaft fracture

目的比较锁定加压钢板固定术与交锁髓内钉固定术治疗肱骨干中段骨折的临床疗效。方法选择长沙市中医院2014年2月—2016年2月收治的肱骨干中段骨折患者78例,随机分为对照组和试验组,各39例。对照组患者采用交锁髓内钉固定术治疗,试验组患者采用锁定加压钢板固定术治疗。比较两组患者术中出血量、手术时间、住院时间、骨折愈合时间及术后并发症发生情况,随访6个月,评估比较两组患者肩关节功能恢复情况。结果两组患者术中出血量、手术时间比较,差异无统计学意义(P>0.05);试验组患者住院时间、骨折愈合时间短于对照组(P<0.05)。试验组患者并发症发生率低于对照组(P<0.05)。试验组患者肩关节功能优良率高于对照组(P<0.05)。结论采用锁定加压钢板固定术治疗肱骨干中段骨折的临床效果优于交锁髓内钉内固定术,其可更有效缩短患者住院时间,促进肩关节功能恢复,且安全性好。

Objective To compare the efficacy of locking compression plate fixation and interlocking intramedullary nail fixation in the treatment of middle humeral shaft fracture. Methods A total of 78 cases of patients with middle humeral shaft fractures were admitted from February 2014 and February 2016 in Traditional Chinese Medicine Hospital of Changsha,which were randomly divided into control group and experimental group,39 cases in each group. The control group was treated with interlocking intramedullary nail fixation, while the experimental group was treated with locking compression plate fixation. The intraoperatve blood loss,surgery duration,hospital stays,fracture healing time and postoperative complications were compared between the two groups. Followed-up for 6 months,recovery of shoulder joints of patients were evaluated. Results There was no statistically significant differences of intraoperatve blood loss or surgery duration between the two group（ P 〉0. 05）. The experimental group of hospital stays and fracture healing time were shorter than experimental group（ P 〈 0. 05）.The rate of complications of experimental group was lower than control group（ P 〈 0. 05）. The good rate of shoulder function recovery of experimental group was higher control group（ P 〈 0. 05）. Conclusion Compared with interlocking intramedullary nail fixation,locking compression plate fixation has a better clinical effect in the treatment of middle humeral shaft fracture,which can shorten hospital stays of patients,helps shoulder joints recovery,and with great safety.
