糖维胶囊联合二甲双胍治疗2型糖尿病胰岛素抵抗患者疗效分析

Efficacy of Tangwei Capsule Combined Metformin Treatment for Type 2 Diabetes Mellitus with Insulin Resistance

目的:观察糖维胶囊联合二甲双胍对2型糖尿病(T2DM)胰岛素抵抗(IR)患者糖脂代谢、抗氧化功能及胰岛素敏感性的影响。方法:根据不同治疗方法将200例T2DM IR患者分为治疗组和对照组,每组各100例,对照组口服二甲双胍,辅以饮食控制及积极运动,治疗组在对照组的基础上加服糖维胶囊,4周为1个疗程。3个疗程后评价疗效,且对治疗前后中医症候总积分、糖脂代谢指标、抗氧化指标及胰岛素敏感指数(ISI)进行测定,并比较组间差异。结果:治疗组治疗总有效率,显著高于对照组(90.00%vs 75.00%,P<0.05);两组治疗后中医症候总积分、空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、丙二醛(MDA)水平均显著下降,ISI、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSHPx)均显著上升,差异有统计学意义(P<0.05);治疗组治疗后上述各指标改善效果均显著优于对照组(P<0.05);两组均无明显不良反应发生。结论:糖维胶囊联合二甲双胍改善T2DM IR患者糖脂代谢紊乱、提高胰岛素敏感度优于单用二甲双胍,其机制可能与其能增强机体抗氧化功能有关。

Objective： To investigate the effect of Tangwei capsule combined with Mettormin on glucose and lipid metabolism, antioxidant function and insulin sensitivity in patients with type 2 diabetes mellitus （T2DM） insu-lin resistance （IR）. Method： 200 cases of patients with T2DM IR＇ were randomly divided into the treatment group and the control group according to different treatment methods. The control group was given routine treatment while the treatment group was given routine treatment and oral administration of Tangwei capsule. 4 weeks was a course of treatment, and the efficacy was evaluated after 3 courses. The total score of TCM symptoms, indexes of glucose and lipid metabolism, antioxidant indexes and insulin sensitivity index （ISI） were determined before and af- ter treatment, and they were compared between groups. Results： The total effective rate of the treatment group was significantly higher than that of the control group （90.00% vs 75.00%） （P〈0.05）. After treatment, the total score of TCM syndrome, fasting plasma glucose （FPG）, 2h postprandial blood glucose （2hPG）, glycosylated hemo-globin （HbAlc）, total cholesterol （TC）, triglyceride （TG）, low-density lipoprotein cholesterol （LDL-C） and ma-londialdehyde （MDA） levels were significantly decreased, while ISI, superoxide dismutase enzyme （SOD） and glutathione peroxidase （GSH-Px） were significantly increased in the two groups （P〈0.05）. After treatment, the above indexes in the treatment group were significantly better than those in the control group （P〈0.05）. There were no obvious adverse reactions. Conclusion： Tangwei capsule combined Metformin can obviously relieve the disorders of glucose and lipid metabolism and improve the insulin sensitivity in patients with T2DM IR. The mechanism may be related to enhancement of the antioxidant function
