摘要
为了掌握河北省2010~2015年手足口病主要病原EV-A71的流行及分子特征,并探讨EV-A71构成情况和重症及死亡病例的相关性,为手足口病的防控提供科学依据。采用Spearman秩相关分析方法对各月度EV-A71构成比分别与各月份全省报告病例数、重症病例数、死亡病例数等指标的关系进行分析,利用DNAstar软件包和MEGA软件分析EV-A71病毒的基因进化特征。2010年1月至2015年12月间,河北省各月份EV-A71构成比与重症病例数、死亡病例数、重症率及病死率之间相关系数分别为0.31、0.38、0.57和0.45(P值均小于0.01)。河北省EV-A71分离株均属C4a亚型。EV-A71是引起河北省大多数手足口病重症和死亡病例的主要病原体。各月份EV-A71构成比与手足口病重症病例数、死亡病例数、重症率及病死率之间存在一定正相关的关系。
We wished to: (i) master the epidemiology and molecular characterization of enterovirus (EV)- A71 pathogens of hand, foot and mouth disease (HFMD) in Hebei Province, China, from 2010 to 2015; (ii) and discuss the relationship between the severe and fatal cases occurring and proportion of EV-A71 ca- ses; (iii) provide evidence for the prevention and control of HFMD in Hebei Province. Rank correlation was used to analyze the relationship between the monthly proportion of EV-A71 cases and incidence, se- vere cases and mortality. The characteristics of genetic evolution of EV-A71 were analyzed by DNAstar and MEGA 5. 2. The correlation coefficients between the monthly proportion of EV-A71 cases and the number of severe cases, number of deaths, the severity ratio and the fatality rate from 2010 to 2015 were 0.31, 0.38, 0.57 and 0. 45(P〈0. 01). The phylogenetic tree showed that the isolates of EV-AT1 in Hebei Province and C4a subtype reference strains were in the same branch. The main pathogen of the HFMD epi- demic was EV-A71, and most severe cases and deaths were caused by EV-AT1. Isolates of EV-A71 in He- bei Province were C4a subtypes. There was no obvious relationship between the sequences of wild-type ca- ses with severe cases and deaths. There was a positive correlation between the monthly proportion of EV- A71 cases and the number of severe cases, number of deaths, severity ratio and fatality rate from 2010 to 2015.
