循证护理在血液高凝状态糖尿病视网膜病变患者中的应用

The Application of Evidence-Based Nursing in Patients with Diabetic Retinopathy in Hypercoagulable State

目的探讨循证护理在血液高凝状态糖尿病视网膜病变患者中的应用研究。方法选取2013年5月~2015年6月我院门诊部收治的82例糖尿病视网膜病变患者为研究对象,依据患者入院顺序将其分为研究组和对照组各41例,对照组实施基础护理干预,研究组另实施循证护理干预,对两组护理干预后临床疗效、血液指标[纤维蛋白原(Fbg)、血清超敏C反应蛋白(hs-CRP)]、纤溶指标[组织纤溶酶原激活物(t PA)、D二聚体(D-D)、纤溶酶原激活抑制物(PAI)]变化及生活质量进行观察比较。结果护理干预后研究组总有效率87.80%较对照组78.05%显著高(P<0.05);护理干预后研究组Fbg、hs-CRP较护理干预前显著降低较对照组显著(P<0.05);护理干预后研究组、t PA,症状与视功能、身体功能、社会活动及精神心理得分较护理干预前显著升高,PAI、D-D显著降低,且均较对照组显著(P<0.05)。结论循证护理在血液高凝状态糖尿病视网膜病变患者中的应用有一定的价值。

Objective To explore the application of evidence-based nursing in patients with diabetic retinopathy in hypercoagulable state. Methods 82 patients with diabetic retinopathy treated in our hospital between May 2013 and June 2015 were divided into study group and control group according to the order of admission,41 cases in each group. The control group was given basic nursing intervention while the study group was given evidence-based nursing intervention. The clinical curative effect, changes of blood indexes[fibrinogen （ Fbg ）, serum high- sensitivity C- reactive protein （ hs- CRP ） ], fibrinolysis indexes ] tissue plasminogen activator（ TPA）, D- dimer（ D- D）, plasminogen activator inhibitor（PAI） ] and quality of life were observed and compared between the two groups after intervention. Results After nursing intervention, the total effective rate of the study group （87.80%） was higher than that of the control group （78.05%） （ P 〈 0. 05 ） ; After nursing intervention, Fbg and hs-CRP in the study group were significantly lower than those before nursing intervention and those in the control group after nursing intervention （ P 〈 0.05 ） ; After nursing intervention,the scores of tPA, symptoms and visual function, physical function, social activities and mental and psychological scores of the study group were significantly higher than those before nursing intervention while PAI and D-D were significantly lower.
