济南市儿童青少年肥胖病并发高血压的中医证型及相关因素研究

Children and Adolescent Obesity Disease complicated with Hypertension TCM Syndrome Type and The Related Factors in Ji'nan

目的:了解儿童青少年肥胖病并发高血压的发病情况,分析本病的影响因素与中医证型的关系,为本病的防治提供依据。方法:收集2015年参加济南市中小学超体重学生"健康夏令营"的306例肥胖患儿信息,通过填写"调查表",选取肥胖病并发高血压患儿69例作为本次研究对象,以分析本病的发病特征、中医证型分布规律及与相关因素的关系。结果:本次调查肥胖患儿306例,其中肥胖病并发高血压患儿69例,在肥胖病中的发病率为22.55%。男生54例(78.3%),女生15例(21.7%),男女比例是3.6∶1。年龄分布为8~14岁,其中10岁患儿居多,为18例(26.09%)。中医证型上以胃热湿阻证多见,为43例(62.3%),其次脾虚湿盛证13例(18.8%),肝郁气滞证9例(13.0%),脾肾两虚证4例(5.8%)。年龄、运动时间与中医证型均有显著差异;性别、年龄、血压等级与证型间均呈负相关(P<0.05)。结论:儿童青少年肥胖病并发高血压在肥胖患儿人群的比例较高,中医证型以胃热湿阻型多见,性别、年龄、血压等级与证型间具有相关性。

Objective： To understand incidence of children and adolescent obesity disease complicated with hypertension disease,analyze the influence factors of the disease and the relationship between TCM syndrome types,and provide the basis for the disease prevention and control.Methods： Collected the information of 306 children who participated in 2015 ＂Healthy Summer Camp＂ in Ji＇nan,selected 69 cases of children obesity disease complicated with hypertension as the research object,by analyzing the characteristic of the onset of the disease and TCM syndrome type distribution and the relationship with the related factors.Results：（1） The survey obese children with 306 cases,69 cases of obesity disease complicated with hypertension,the incidence of obesity is 22.55%.There are 54 boys（78.3%）,15 girls（21.7%）,and the ratio of boy and girl is 3.6：1.The onset age is focused on 10 to 13 years old.It contains 18 cases（26.09%） in 10 years old,（2） The distribution of TCM syndrome type is stomach heat and dampness retention syndrome in 43 cases（62.3%）,spleen dampness syndrome in 13 cases（18.8%）,liver qi stagnation syndrome in 9 cases（13.0%）,spleen and kidney deficiency syndrome in 4 cases（5.8%）.（3） Four syndromes and related factors can be analyzed that there are statistically significant difference（P〈0.05） in age,exercise time and sleep habits and gender,age and blood pressure levels are negatively correlated with four syndromes.Conclusions：（1） Child and adolescent obesity disease complicated with hypertension in a higher percentage of obese people.（2） The most common type is the stomach heat and dampness retention syndrome in children and adolescents obesity complicated with hypertension.（3） Gender,age and blood pressure levels are negatively correlated with four syndromes.
