摘要
目的探究地佐辛辅助镇痛应用于重症加强护理病房(ICU)肺癌手术后的效果。方法 60例肺癌患者,随机分为对照组与观察组,每组30例。对照组于术后应用自控镇痛泵泵入舒芬太尼,观察组于术后应用自控镇痛泵泵入地佐辛辅助镇痛。对比两组的镇痛效果、并发症发生情况。结果术后8、12、24、48 h两组视觉模拟评分法(VAS)评分均低于术后4 h,组内比较差异均具有统计学意义(P<0.05);术后4、8、12、24、48 h两组VAS评分组间比较差异均无统计学意义(P>0.05)。术后4、24、48 h,观察组血糖水平(6.60±0.93)、(8.20±2.30)、(9.20±1.50)mmol/L均低于对照组(9.30±2.85)、(12.80±3.74)、(11.90±2.91)mmol/L,差异均具有统计学意义(P<0.05)。观察组镇痛相关并发症发生率6.67%明显低于对照组的30.00%,差异具有统计学意义(P<0.05)。结论在肺癌患者术后辅助镇痛中应用地佐辛不仅能取得满意的镇痛效果,同时还能在一定程度上缓解患者的术后炎症应激反应,其用药安全性较高,能够提高患者的术后舒适度,值得推广。
Objective To investigate the effect of dezocine assisted analgesia applied in postoperative intensive care unit(ICU) lung cancer surgery. Methods A total of 60 lung cancer patients were randomly divided into control group and observation group, with 30 cases in each group. The control group received treated with sufentanil by self-controlled analgesia pump, and the observation group received dezocine assisted analgesia by self-controlled analgesia pump. Comparison were made on analgesic effect and complications between two groups. Results In postoperative 8, 12, 24 and 48 h, both groups had lower visual analogue scale(VAS) score than that of postoperative 4 h, and their difference was statistically significant(P0.05). In postoperative 4, 8, 12, 24 and 48 h, both groups had no statistically significant difference in VAS score(P0.05). In postoperative 4, 24 and 48 h, the observation group had lower blood glucose level as(6.60±0.93),(8.20±2.30) and(9.20±1.50) mmol/L than(9.30±2.85),(12.80±3.74) and(11.90±2.91) mmol/L in the control group, and their difference was statistically significant(P0.05). The observation group had obviously lower analgesia-related complication rate as 6.67% than 30.00% in the control group, and the difference was statistically significant(P0.05). Conclusion Application of dezocine assisted analgesia in postoperative lung cancer surgery patients not only can achieve satisfactory analgesic effect, but also can alleviate patient's postoperative inflammation emergency response to some extent. It has high medication safety and can improve the comfort of patients after surgery. It is worth promoting.
