河南省终末期肾病透析患者疾病经济负担研究

Analysis of direct economic burden of dialysis in patients with end stage renal disease in Henan Province

目的通过对河南省腹膜透析试点医院的终末期肾病患者透析治疗费用调查,比较不同透析治疗方法患者疾病经济负担差异。方法以2014年河南省腹膜透析试点医院,终末期肾病透析治疗的患者为研究对象,采取整群随机抽样的方法抽样,通过随诊问卷调查获得各透析患者的平均每年的疾病经济负担,并进行相关分析。结果血液透析的直接医疗费用为70 712.00元,腹膜透析为58 261.50元,血透患者高于腹透患者(Z=-5.876,P<0.001);主要直接非医疗费用中,血透患者营养费为1 539.00元,腹透患者为1 140.00元,血透患者高于腹透患者(Z=-2.385,P<0.017),血透患者就医交通费为1 013.00元,腹透患者为982.00元,血透患者高于腹透患者(Z=-1.092,P<0.275)。结论对于终末期肾病患者而言,采用腹膜透析治疗,可以缩减救治费用,减轻经济负担。

Objective To analyze and compare the economic burden of nephrosis patients at terminal stage in hemodialysis and peritoneal dialysis treatment in Henan province. Methods Obtain the economic burden of the ESRD dialysis patients by questionnaire and follow-up with the help of pilot hospital. Calculate and analyze the economic burden of the disease through the method of economic burden calculation in multiple centers. Results The direct medical costs of hemodiatysis and peritoneal dialysis were 70712. 00 RMB and 58261. 50 RMB per patient per year. The costs of hemodialysis patients were higher than that of peritoneal dialysis patients,and there were statistical significance between them（ Z=-5. 876,P〈0. 001）. In the main non-medical costs,hemodialysis patients＇ nutrition fee was 1539. 00 RMB,higher than peritoneal dialysis patients＇ 1140. 00 RMB,（ Z =-2. 385,P〈0. 017）. In terms of transportation cost,the former one was 1013 RMB,while the latter one was 982 RMB（ Z =-1. 092,P〈0. 275）. Conclusion The patients with end stage renal disease can choose peritoneal dialysis treatment to save medical costs and lighten the economic burden.
