超声下未成熟卵泡抽吸术治疗多囊卵巢综合征不孕症的疗效及对内分泌功能的影响

Effect of IMFA on the efficacy and endocrine function of PCOS infertile patients under ultrasound

目的探讨超声下未成熟卵泡抽吸术(immature follicle aspiration,IMFA)治疗多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕症患者的疗效及对患者内分泌功能的影响。方法选取2013年1月-2015年1月在湖南省妇幼保健院治疗的94例PCOS不孕症患者,采用随机数字表分为IMFA组(IMFA治疗)、对照组(采用达英-35治疗)各47例,对比两组的疗效及内分泌相关指标。结果治疗后,IMFA组患者的血清中睾酮(Testosterone,T)、黄体生成素(luteinizing hormone,LH)均低于对照组,卵泡刺激素(follicle stimulating hormone FSH)高于对照组,差异均具有统计学意义(P<0.05),两组患者的空腹胰岛素(fasting insulin,INS)水平差异无统计学意义(P>0.05),IMFA组患者的卵巢体积、阻力指数(resistance index,RI)均低于对照组,卵巢动脉最大血流速度(maximum blood flow velocity,Vmax)高于对照组,差异均具有统计学意义(P<0.05),IMFA组患者的月经恢复率63.83%、妊娠率44.68%均高于对照组的36.17%、23.40%(P<0.05)。结论超声下IMFA治疗PCOS不孕症患者能更显著的改善患者内分泌水平、促进卵巢形态及功能恢复、提高患者的月经恢复及妊娠率。

Objective To investigate the curative effect and the effect on endocrine function of ultrasound-guided immature follicle aspiration （IMFA） on infertility patients with polycystic ovary syndrome （PCOS）. Methods From January 2013 to January 2015, 94 infertility patients with PCOS treated in our hospital, were randomly divided into IMFA group （IMFA treatment） and control group （ cured by Diane - 35） 47 cases, and compared the curative effect and endocrine related indicators in the two groups. Results After treatment, compared with control group, the patient in IMFA group had lower levels in serum Testosterone and Luteinizing hormone, and higher level in Follicle stimulating hormone, and the differences were statistically significant （ P 〈 0.05 ）. The difference was no statistically significant in the level of INS between the two groups （ P 〉 O. 05 ）. Compared with control group, IMFA group had lower ovarian volume and resistance index, and higher ovarian artery maximum blood flow velocity, and the differences were statistically significant （P 〈 0.05）. The menstrual recovery rate of IMFA group was 63.83%, and the pregnancy rate was 44.68%, these indexes were higher than those in control group （ 36. 17% and 23.40% ） （P 〈 0.05）. Conclusion The treatment of ultrasound-guided IMFA on infertility patients with PCOS can significantly improve the endocrine level, promote the recovery of ovarian morphology and function, and improve the rate of menstrual recovery and pregnancy.
