摘要
目的探讨肺癌患者经皮微波消融术前后外周血清血管内皮生长因子(VEGF)和血管形成素-2(Ang-2)表达的水平及意义。方法选取2012年1月至2016年4月间解放军第401医院收治的行经皮微波消融术的73例肺癌患者为研究组,另选取同期50例健康体检者为对照组。观察研究组患者治疗有效率,采用酶联免疫吸附法(ELISA)测定对照组与研究组治疗前、治疗后3d、7d及14d外周血清中VEGF和Ang-2水平,将研究组各时段的VEGF和Ang-2水平与对照组比较,并分析VEGF和Ang-2水平的相关性。结果研究组患者经皮微波消融术后治疗有效率为82.2%(60/73);研究组患者治疗前外周血清中VEGF和Ang-2水平均高于对照组,差异均有统计学意义(均P<0.05)。经Pearson积矩相关分析,研究组患者治疗前外周血清中VEGF和Ang-2水平呈正相关,差异有统计学意义(r=0.691,P<0.05)。研究组患者外周血清中VEGF和Ang-2水平随治疗时间的推移,先增加后降低,其中治疗后3d、7d高于治疗前,治疗后14d低于治疗前,差异均有统计学意义(均P<0.05)。且研究组治疗后14d,VEGF和Ang-2水平均高于对照组,差异均有统计学意义(均P<0.05)。结论微波消融术治疗肺癌具有较好的临床疗效,通过降低外周血清中VEGF和Ang-2水平抑制肿瘤细胞的生长和转移,联合检测VEGF和Ang-2水平有助于肺癌的诊断。
Objective To explore the expression and significance of serum vascular endothelial growth factor( VEGF) and angiopoietin-2( Ang-2) levels in peripheral blood of patients with lung cancer before and after percutaneous microwave ablation( PMWA). Methods A total of 73 patients with lung cancer undergoing PMWA was selected as the study group and 50 healthy individuals who underwent physical examination were considered as the control group at The 401 th Hospital of PLA from January 2012 to April 2016. The treatment efficiency was observed. The peripheral serum levels of VEGF and Ang-2 before treatment and at 3 d,7 d and 14 d after treatment were detected by the enzyme-linked immunosorbent assay( ELISA) and the relationship between VEGF and Ang-2 was analyzed. Results The efficacy rate of PMWA was 82. 2%( 60/73) in the study group. The peripheral serum levels of VEGF and Ang-2 were higher in the study group than the control group before the treatment( all P〈0. 05). The Pearson product-moment correlation coefficient analysis showed peripheral serum VEGF was positively correlated with Ang-2 in the study group( r = 0. 691,P〈0. 05). The peripheral serum levels of VEGF and Ang-2 increased at first and then decreased with the time,with 3 d and 7 d after treatment higher than before the treatment and 14 d after treatment lower than before the treatment( all P〈0. 05). The above two indexes at 14 d after treatment were higher in the study group than the control group( all P〈0. 05). Conclusion PMWA in the treatment of lung cancer has high efficacy rate,which can inhibit the growth and metastasis of tumor cells by decreasing the level of peripheral serum VEGF and Ang-2. Combined detection of VEGF and Ang-2 levels is helpful for the diagnosis of lung cancer.
