单核细胞及中性粒细胞/淋巴细胞比值与中晚期肿瘤的相关性

Relationship between neutrophil lymphocyte ratio and monocyte count with advanced malignant tumor

目的探讨中性粒细胞淋巴细胞比值(NLR)、单核细胞计数(MONO)与中晚期肿瘤的关系。方法回顾性分析2015年1月~2016年1月武汉市第三医院住院患者193例的临床资料,分为肿瘤组(102例)和非肿瘤对照组(91例),同时按照KPS评分标准将肿瘤组分为病情稳定组(53例)和病情重危组(49例),并分析单核细胞及中性粒细胞/淋巴细胞比值与中晚期肿瘤的相关性。结果肿瘤组的MONO和NLR水平显著高于对照组(P<0.01);病情稳定组的NLR水平低于重危组(P<0.05),而两者的MONO值差异无统计学意义(P>0.05)。高水平的MONO值及NLR是肿瘤发生的危险因素[OR=0.072,95%CI=0.012~0.43,P=0.004;OR=0.389,95%CI=10.212~0.712,P=0.002]。NLR、MONO值预测肿瘤发生的ROC曲线下面积分别为0.787、0.629。结论 MONO及NLR增高与肿瘤发生显著相关。

Objective To investigate the relationship between neutrophil lymphocyte ratio （NLR）, monocyte （MONO） count and advanced tumor. Methods Clinical data of 193 hospitalized patients in Wuhan Third Hospital from January 2015 to January 2016 were retrospectively analyzed, and they were divided into cancer group （102 cases） and non-tumor control group （91 cases）, at the same time, the tumor group was divided into stable condition group （53 cases） and- critically ill group （49 cases） according to KPS score. The relationship between monocyte and neutrophil/lymphocyte ra-tio and advanced tumor was analyzed. Results The levels of MONO and NLR in the tumor group were significantly higher than those in the control group （P 〈 0.01）. The NLR level in the stable group was lower than that in the severe risk group （P 〈 0.05）, there was no significant difference in MONO between the two groups （P 〉 0.05）. High levels of MONO and NLR were the risk factors for tumorigenesis [OR = 0.072, 95%CI = 0.012-0.43, P = 0.004; OR = 0.389, 95%CI = 10.212-0.712, P = 0.002], the area of ROC curve of NLR, MONO values in prediction of tumorigenesis were 0.787, 0.629. Conclusion The increase of MONO and NLR is significantly correlated with tumorigenesis.
