益脾芪连汤治疗气阴两虚兼血瘀型糖尿病肾病临床研究

Clinical Study on Yipi Qilian Decoction in the Treatment of DN: Deficiency Syndrome of Both Qi and Yin Complicated with Blood-stasis

目的:观察益脾芪连汤治疗气阴两虚兼血瘀型糖尿病肾病的临床疗效。方法:选取符合纳入标准的110例气阴两虚兼血瘀型糖尿病肾病患者为研究对象,随机分为研究组和对照组,各55例。所有患者均接受糖尿病基础治疗,对照组加服渴络欣胶囊治疗,研究组加服益脾芪连汤治疗。比较两组患者治疗前后临床症状、24 h尿蛋白定量、尿白蛋白排泄率、尿素氮、血肌酐的变化情况,评价两组患者临床疗效及安全性。结果:研究组临床疗效总有效率为92.73%(51/55),高于对照组72.73%(40/55),差异具有统计学意义(P<0.05);与治疗前比较,两组患者中医症候积分、24 h尿蛋白定量、尿白蛋白排泄率在治疗后均下降,且研究组下降幅度大于对照组,差异具有统计学意义(P<0.05);与治疗前比较,两组患者尿素氮、血肌酐水平在治疗后均未见明显变化,差异无统计学意义(P>0.05);两组患者在治疗过程中均未出现不良反应。结论:采用益脾芪连汤治疗气阴两虚兼血瘀型糖尿病肾病可显著提高临床疗效,缓解临床症状,降低24 h尿蛋白定量,且安全无毒副作用,促进患者身体恢复,优势显著,具有临床推广意义。

Objective： To observe the clinical efficacy of Yipi Qilian decoction in the treatment of diabetic ne- phropathy（DN） of deficiency syndrome of both qi and yin complicated with blood -stasis. Methods： 110 pa- tients met the criterion were randomly divided into the observation group （ n = 55 ） and the control group （ n = 55）. The patients all received basic treatment for DN, on which basis, the control group was given Keluoxin capsules; whereas, the observation group was treated with Yipi Qilian decoction. The changes of clinical symptoms, 24 h urine protein quantity （24 h UPQ）, urinary albumin excretion rate （UAER）, blood urea nitro- gen （BUN）, serum creatinine（Scr） were compared before and after the treatment in the two groups, and clini- cal efficacy and safety of the two medications were evaluated. Results： The total effective rate of the observa- tion group was 92.73% （51/55） , which was significantly higher than 72.73% （40/55） of the control group （P 〈 0.05 ） ; the scores of TCM symptom, 24 h UPQ, UAER were obviously decreased after the treatment compared to those before the treatment in the two groups, and the dropping degree was more apparently in the observation group than that of the control group （P 〈 0.05 ）. However, there was no statistical difference be- tween before the treatment and after the treatment in terms of the levels of BUN and Scr in the both groups （P 〉 0. 05）. There was no obvious adverse reactions in the two groups during the treatment. Conclusion： The therapy of Yipi Qilian decoction can significantly increase clinical efficacy of DN with deficiency syndrome of both qi and yin complicated with blood - stasis, relieve clinical symptoms, decrease 24 h UPQ, promote patients ＂recovery with its safe and non-toxic advantage, which has remarkable significance of clinical promotion.
