骶四针疗法联合补脾固活汤治疗产后压力性尿失禁患者52例

Bushen Guhuo Decoction Combined with Sacral Four-needle Therapy in the Treatment of Postpartum Stress Urinary Incontinence for 52 Cases

目的探究骶四针疗法与补脾固活汤联合治疗对产后压力性尿失禁患者盆底肌功能恢复及生活质量的影响。方法选取产后压力性尿失禁患者103例,依据治疗方法不同分组,对照组51例予以骶四针疗法,研究组52例于对照组基础上加用补脾固活汤治疗,比较两组治疗前后国际尿失禁咨询委员会尿失禁问卷表简表(ICI-Q-SF)评分、尿失禁量,对比两组治疗前后盆底肌功能指标(阴道收缩持续时间、阴道静息压(VRP)、阴道收缩压(VSP))及生活质量(SF-36)评分,统计两组临床效果。结果研究组治疗1个月后尿失禁量少于对照组,ICI-Q-SF评分低于对照组,差异有统计学意义(P<0.05);研究组临床效果优于对照组,差异有统计学意义(P<0.05);研究组治疗1个月后阴道收缩持续时间长于对照组,VRP、VSP高于对照组,差异有统计学意义(P<0.05);研究组治疗1个月后SF-36评分高于对照组,差异有统计学意义(P<0.05)。结论骶四针疗法与补脾固活汤联合治疗产后压力性尿失禁患者效果显著,可有效改善其盆底肌功能及尿失禁症状,有助于提升其生活质量。

Objective To investigate the effect of sacral four-needle therapy combined with Bushen Guhuo decoction on pelvic floor muscle function recovery and quality of life in postpartum stress urinary incontinence patients. Methods 103 patients with postpartum stress urinary incontinence were selected and divided into control group（ n = 51） and study group（ n = 52） according to different treatment methods. The control group was treated with sacral four-needle therapy,and the study group was given Bushen Guhuo decoction on the basis of the control group. International Urinary Incontinence Advisory Board Urinary Incontinence Questionnaire（ ICI-Q-SF） score and urinary incontinence were compared between the two groups before and after treatment,the pelvic floor muscle function index [vaginal contraction duration,vaginal resting pressure（ VRP）,vaginal systolic pressure（ VSP） ] and quality of life（ SF-36） score was compared,and the clinical effect was statistically analyzed. Results Urinary incontinence in the study group was less than that of the control group within one month after treatment,and ICI-Q-SF score of the study group was lower than that of the control group,and the difference was statistically significant（ P〈0. 05）. The clinical effect of the study group was better than that of the control group,and the difference was statistically significant（ P〈0. 05）. The duration of vaginal contraction in the study group was longer than in the control group within one month after treatment,and the VRP and VSP in the study group were higher than those in the control group,and the difference was statistically significant（ P〈0. 05）. The SF-36 score in the study group was significantly higher than that of the control group after one month（ P〈0. 05）. Conclusion The sacral four-needle therapy combined with Bushen Guhuo decoction in the treatment of postpartum stress urinary incontinence gets a significant effect,which can improve the pelvic floor muscle function and urinary incontinence symptoms,and improve the quality of life.
