髋关节置换术结合桃红四物汤治疗髋关节疾病的临床疗效观察

Observation on the Clinical Curative Effect of Hip Replacement Combined with Taohong Siwu Decoction in the Treatment of Hip Joint Disease

目的观察髋关节置换术结合桃红四物汤治疗髋关节疾病的临床疗效。方法选取于本院行髋关节置换术的患者90例,随机分为观察组(45例)与对照组(45例),对照组术后给予常规治疗,观察组在对照组的基础上结合桃红四物汤治疗,两组均治疗2周。治疗后观察两组患者深静脉血栓(DVT)发生情况,检测并比较两组患者术后总出血量、治疗后VAS评分及术后总引流量;比较两组患者术后肿胀消退时间。结果治疗后观察组DVT发生率明显低于对照组(P<0.01);观察组术后总出血量、VAS评分及总引流量均低于对照组(P<0.01);治疗后观察组肿胀消退时间为2~6 d的例数多于对照组,而肿胀消退时间为6~10 d、10~14 d的例数少于对照组(P<0.01),观察组肿胀平均消退时间较对照组明显缩短(P<0.01)。结论髋关节置换术可切除病灶,恢复病变关节的活动与功能,术后结合桃红四物汤可有效减少髋关节疾病患者术后DVT的发生,降低其疼痛程度,促进创口愈合,缓解患者术后肢体肿胀,临床疗效显著。

Objective To observe the clinical curative effect of hip replacement combined with Taohong Siwu decoction in the treatment of hip joint disease. Methods 90 cases of hip joint disease patients who underwent hip replacement in our hospital were selected and divided randomly into observation group（ n = 45） and control group（ n = 45）. The control group was treated with routine therapy after operation.The observation group was treated combined with Taohong Siwu decoction based on the control group. Two groups were treated for 2 weeks.The occurrence of the deep vein thrombosis（ DVT） in 2 groups of patients after treatment was observed,the postoperative blood loss,VAS score after treatment and total discharge after operation,the swelling extinction time after treatment of 2 groups was examined and compared.Results The DVT incidence in the observation group was significantly lower than that in the control group（ P〈0. 01）. The postoperative blood loss,VAS score after treatment and total discharge after operation of observation group were lower than those of the control group（ P〈0. 01）. After treatment,the proportion of patients whose swelling extinction time were 2 ~ 6 d after treatment in the observation group were significantly more than those in the control group,the proportion of patients whose swelling extinction time 6 ~ 10 d and 10 ~ 14 d in the observation group were less than those in the control group（ P〈0. 01）. The average swelling extinction time of the observation group shortened significantly comparing with the control group（ P〈0. 01）. Conclusion The Hip replacement can extract the lesion,recover the activity and function of the diseased joint,and Taohong Siwu decoction can effectively reduce the DVT occurrence of patients with hip disease,improve the patients＇ blood coagulation function,relieve the pain and inflammation,and significantly improve the clinical effect.
