某医院2015-2016年新生儿神经行为评分异常发生情况及影响因素分析

Analysis of Abnormal Neonatal Neurobehavioral Scores and Influencing Factors in a Hospital from 2015 to 2016

目的探讨新生儿神经行为评分(NBNA)异常发生情况及影响因素。方法选取2015年1月-2016年12月我院收治的186例新生儿作为研究对象,采用回顾性分析法分析所有新生儿的临床资料,根据资料结果记录所有新生儿和其母亲的基本情况以及新生儿NBNA评分情况,分析新生儿NBNA评分异常的发生情况,并比较不同NBNA评分新生儿间其上述资料的差异,研究影响新生儿NBNA评分异常的相关因素。结果本组研究186例新生儿中NBNA评分异常发生率为52例(27.96%);非条件单因素Logistic回归模型分析显在2组新生儿中胎龄、出生体质量、小于胎龄儿、多胎、新生儿窒息、呼吸窘迫综合征、颅内出血、高胆红素血症、高脑钠肽以及新生儿母亲高血压及糖尿病的发生情况比较差异间均具有统计学意义(P<0.05);通过非条件多因素Logistic回归模型分析可得,胎龄、出生体质量、新生儿窒息、颅内出血、高胆红素血症、呼吸窘迫综合征以及高脑钠肽均为导致新生儿NBNA评分异常的独立危险因素(P<0.05)。结论新生儿NBNA评分异常发生率较高,其中胎龄、出生体质量、新生儿窒息、颅内出血、高胆红素血症、呼吸窘迫综合征以及高脑钠肽均为导致新生儿NBNA评分异常的独立危险因素。

Objective To investigate the abnormalities of neonatal neurobehavioral NBNA scores and the influencing fac-tors. Methods A total of 186 newborns admitted to our hospital between January 2015 and December 2016 were selected as sub-jects. The clinical data of all these newborns was analyzed retrospectively. Basic information of all these newborns and their moth- ers and the neonatal neurobehavioral scores were recorded according to the data. The incidence of neonatal neurobehavioral abnor-malities was analyzed and the above data was compared between newborns with different neurobehavioral scores. The factors that influenced the abnormalities of neonatal neurobehavioral scores were investigated. Results The incidence of abnormal NBNA scores among these 186 newborn infants was 27.96% （52 cases） . Univariate single factor logistic regression analysis showed that significant difference was detected in gestational age,birth weight,short gestational age,multiple births,neonatal asphyxia,respira-tory distress syndrome,intracranial hemorrhage,hyperbilirubinemia,high brain natriuretic peptide,hypertension of neonatal moth-ers and diabetes between the two groups of newborns （ P〈0. 05 ）. Analysis by the unconditional multi - factor logistic regression model suggested that pregnancy, birth weight, neonatal asphyxia, intracranial hemorrhage, hyperbilirubinemia, respiratory distress syndrome 肌d high brain natriiffetic peptide were aQ independent risk factors for abnormal neonatal NBNA scores （P〈0.05 ） . Conclusion The incidence of abnormal neonatal NBNA scores is high. The gestational age, birth weight, neonatal asphyxia, in-tracranial hemorrhage,hyperbilirubinemia,respiratory distress syndrome and high brain natriuretic peptide are all independent riskfactors for neonatal NBNA score abnormalities.
