三维彩色能量多普勒超声对胎盘植入的诊断价值研究

Value of three dimensional color power Doppler ultrasound in diagnosis of placenta accrete

目的:探讨三维彩色能量多普勒超声(three-dimensional color power Doppler ultrasound,3D-PDU)立体重建技术对胎盘植入的诊断价值。方法:收集65例可疑胎盘植入患者作为研究对象,均进行3D-PDU检查,并将3DPDU所得诊断结果与"金标准"(临床和/或病理诊断)进行比较。结果:3D-PDU检查与"金标准"诊断结果比较无显著差异(P=0.754),2种诊断方法的吻合度具有一致性(Kappa=0.661,P<0.001)。结论:3D-PDU与三维立体重建技术相互结合,可更直观、形象地显示病变处立体结构、血流灌注情况、血管空间位置情况,并能够提供量化指标,可有效判断胎盘植入深度、面积,对胎盘植入早期诊断及治疗方案制订、预后评估均具有重要意义。

bObjective To explore the diagnostic value of three-dimensional color Doppler ultrasonography（3 D-PDU） threedimensional reconstruction technique for placenta accrete.Methods Totally 65 cases of suspected patients with placenta implantation underwent 3 D-PDU examination, then the examination results were compared with those by clinical or pathological method, and the results by clinical or pathological method were used as the gold standard.Results There were no significant differences between the results by 3 D-PDU and the gold standard（P =0.754）, and there was consistency between the two methods（Kappa =0.661,P0.001）.Conclusion 3 D-PDU combined with 3 D reconstruction technique contributes to displaying lesion structure, blood perfusion and etc and provides quantitative indexes to determine the depth and size of the accreted placenta, and thus is of great significance for the early diagnosis and treatment planning of the placenta accrete.
