介入治疗急性下肢深静脉血栓形成的疗效分析

Effects of interventional therapy for acute deep venous thrombosis of lower limbs

目的研究介入治疗急性下肢深静脉血栓形成的效果。方法选取2015年8月至2016年8月在本院进行治疗的急性下肢深静脉血栓形成(DVT)患者79例,按照奇偶数分为对照组和观察组。对照组39例,采取血管腔内介入治疗,观察组40例,采取导管接触性溶栓术治疗。对比两组的膝上、下15 cm处健、患肢周径差与小腿和大腿周径差及消肿率、临床疗效、并发症发生率、住院时间。结果观察组急性下肢DVT患者治疗后的膝上、下15 cm处健、患肢周径差以及小腿和大腿周径差均少于对照组患者(P<0.05),小腿和大腿消肿率、治疗优良率高于对照组(P<0.05),组间对比并发症发生率差别不大(P>0.05),观察组患者的住院时间短于对照组(P<0.05)。结论在急性下肢DVT患者的治疗中应用导管接触性溶栓术进行介入治疗安全有效,具有较高的临床推广价值。

Objective To study the efficacy of interventional treatment of acute deep vein thrombosis of lower extremities. Methods From Aug. 2015 to Aug. 2016, 79 cases acute deep vein thrombosis of lower extremity were analyzed retrospectively. The patients were divided into 2 groups according to odd or even number. In 79 cases, 39 cases were treated with endovascular interventional therapy（control group）, 40 cases were treated with catheter directed thrombolysis（observation group）. The perimeter differences of the healthy side and the affected side were compared at the 15 cm up/below knee joint, calf and thigh, as well as the clinical efficacy, complication rate and hospital stay. Results Compared with the control group, the perimeter differences of the healthy side and the affected side at the 15 cm up/below knee joint, calf and thigh were significantly lower in observation group（P〈0.05）, the swelling release rate of calf and thigh, as well as the clinical efficacy were significantly better in the observation group versus control group（P0.05）.The complication rate was not significantly different between the two groups（P0.05）.The hospital stay of the observation group was shorter than that of the control group（P0.05）. Conclusion Treatment of acute deep vein thrombosis with catheter-induced thrombolysis is safe and effective, with high clinical value.
