双源CT双能量成像技术诊断痛风尿酸盐结晶的临床价值研究 被引量:10

摘要 目的探讨双源CT双能量成像技术诊断痛风患者尿酸盐结晶的临床价值。方法选取2016年6—12月丽水市中心医院疑似痛风患者120例,按照国际公认美国风湿病学会(ACR)的痛风分类标准将其分为痛风组(n=75)和非痛风组(n=45)。两组均进行双源CT双能量成像技术检查,观察两组尿酸盐结晶阳性率;并分析双源CT双能量成像技术的灵敏度、特异度和准确率。结果痛风组尿酸盐结晶阳性率高于非痛风组(P<0.05);75例痛风患者中,尿酸盐沉积于双手腕关节34例(45.3%)、双足踝关节26例(34.7%)、双足15例(20.0%)。双源CT双能量成像技术诊断痛风患者尿酸盐沉积的灵敏度为93.3%,特异度为94.7%,准确率为96.0%。结论双源CT双能量成像技术能准确诊断出尿酸盐沉积部位,可作为痛风病筛查的辅助手段。 Objective To investigate the clinical value of dual-source CT dual energy imaging in the diagnosis of uric acid crystals in gout patients. Methods 120 cases with suspected gout in Central Hospital of Lishui from June to December in2016 were divided into gout group( n = 75) and non-gout group( n = 45) according to the internationally accepted American College of Rheumatology( ACR). Dual-source CT dual energy imaging technique was used to observe the incidence of urinary acid deposition,and the sensitivity and specificity of the two groups were compared. Results The positive rate of urinary acid crystals in gout group was significantly higher than that in non-gout group( P < 0. 05). Among the 75 cases with gout,the urate was deposited in wrist joint of 34 cases( 45. 3%), ankle of 26 cases( 34. 7%), feet 15 cases( 20. 0%). The sensitivity of dual-source CT dual energy imaging in the diagnosis of urinary acid deposition was 93. 3%, specificity was94. 7%,and the accuracy was 96. 0%. Conclusion Dual-source CT dual-energy imaging can accurately diagnose uric acid deposition sites and be used as an adjunct to gout screening.
