常州地区基本医疗保险制度满意度及其影响因素研究

Study of satisfaction and influencing factors of basic medical insurance system in Changzhou city

目的探讨常州市参保居民对基本医疗保险制度的满意度及其影响因素。方法随机抽取常州市新北区2014年参保居民358名进行问卷调查,358名参保人员来自常州四院、孟沙人民医院、春江人民医院、薛家卫生院、河海社区卫生服务中心、龙虎塘社区卫生服务中心、三井街道社区卫生服务中心7家单位,调查分析参保居民对基本医疗保险制度满意度及其影响因素。结果与城镇职工和城镇居民基本医疗保险的参保人员的满意度评分相比,新农合的参保人员的满意度评分较低。医疗服务态度、报销起付线、报销比例、医生技术水平、政策了解程度、转诊方便性、参保个人缴费水平和医疗设备状况是城镇职工基本医疗保险制度满意度的影响因素(P<0.05)。交通便利性、报销起付线、报销比例、医生技术水平、转诊方便性、参保个人缴费水平和医疗设备状况是城镇居民基本医疗保险参保者的满意度的影响因素(P<0.05)。交通便利性、报销起付线、报销比例、医生技术水平、转诊方便性和医疗设备状况时新农合保险制度满意度的影响因素(P<0.05)。结论基本医疗保险制度的参保者对于医疗技术水平(医疗设备状况和医生技术水平)和医疗费用(报销水平和比例,个人缴费水平)关注程度较高,医疗技术水平和医疗费用的高低也是影响参保者对基本医保制度满意度的重要影响因素。

Objective To investigate the satisfaction and influencing factors of basic medical insurance system in Changzhou City. Methods 358 insurance residents who were from Changzhou Four Courtyard, Mengsha People’ s Hospital,Spring River People’s Hospital,Celia Health Center,Hohai Community Health Service Center,Tiger Community Health Service Center,Three Well Street Community Health Service Center Seven Units in 2014, the residents’ satisfaction with the basic medical insurance system and its influencing factors were investigated and analysed. Results Compared with satisfaction on the urban employees and basic medical insurance system insurance residents, the insurance residents from new rural cooperative medical system were lowest. Medical service attitude,reimbursement of the pay lines,the reimbursement ratio,the medical level of doctors,the level of policy understanding, referral of convenience, personal payment levels and medical equipment were influencing factors of satisfaction of urban employees（ P 〈 0. 05）. Ease of transport, reimbursement of the pay lines, the reimbursement ratio,the medical level of doctors,ease of referral,personal contribution levels,and medical equipment were influencing factors of satisfaction of basic medical insurance system insurance residents（ P 〈 0. 05）. Ease of transport,reimbursement of the pay lines,the reimbursement ratio,the medical level of doctors,ease of referral,and medical equipment were influencing factors of satisfaction of new rural cooperative medical system（ P 〈 0. 05）. Conclusion People who participated in the basic medical insurance system mostly concerned about the level of medical technology（ medical equipment and doctor’s medical level） and medical expenses（ reimbursement level and proportion,personal payment levels）. The most important factors which affected the satisfaction of basic medical insurance are the level of medic care and the medical expense.
