同型半胱氨酸、转铁蛋白及N-乙酰-β-D-葡萄糖苷酶联合检测对早期糖尿病肾病的临床诊断价值 被引量:8

The combined detection value of homocysteine,transferrin and N-acetyl beta-D-glucosidase in the diagnosis of early diabetic nephropathy

摘要 目的观察Cys-C、NAG和TRF 3项生化指标联合检测对糖尿病肾病的早期诊断价值。方法 2014年2月-2016年12月本院收治的单纯糖尿病患者80例为单纯糖尿病组,并收集糖尿病肾病患者80例为糖尿病肾病组,另收集同期本院体检健康者80例为对照组,观察比较3组研究对象的Cys-C、TRF和NAG水平和阳性率,比较单独检测与联合检测在诊断糖尿病肾病中的效率。结果糖尿病组患者的Cys-C、TRF和NAG的含量均明显高于对照组,糖尿病肾病患者组的Cys-C、TRF和NAG血清中的含量均明显高于对照组和糖尿病组,上述差异均有统计学意义(P<0.05);糖尿病肾病患者组中的Cys-C、TRF和NAG阳性率均明显高于其他2组,3组间差异均有统计学意义(P<0.05);3项生化指标联合检测结果对糖尿病肾病灵敏度及特异度均明显高于单一检测的结果,差异有统计学意义(P<0.05)。结论Cys-C、TRF和NAG 3项生化指标联合检测对糖尿病肾病早期诊断有重要的临床意义。 Objective To study the value of the combined detection of homocysteine( Cys-C),N-acetyl-D-( NAG) and transferrin( TRF) in the early diagnosis of diabetic nephropathy( DN). Methods 80 cases of diabetic patients,80 cases of diabetic nephropathy patients and 80 cases of healthy subjects were respectively selected as the single diabetic group,the diabetic nephropathy group and the control group. The Cys-C,TRF and NAG levels as well as the positive rates of the 3 groups were compared,and the efficiency of single detection and combined detection of Cys-C,TRF and NAG for the diagnosis of diabetic nephropathy were compared. Results The levels of Cys-C,TRF and NAG in the diabetic group were significantly higher than the control group,but the levels of Cys-C,TRF and NAG in the diabetic nephropathy group were significantly higher than those in control group and diabetic group,and the differences were statistically significant( P 〈0. 05); the positive rates of Cys-C,TRF and NAG in the diabetic nephropathy group were significantly higher than that in the diabetic group and the control group,and the differences among the 3 groups were statistically significant( P 〈0. 05); specificity and sensitivity of the combined detection of Cys-C,TRF and NAG on diabetic nephropathy were significantly higher than the single detection,and the differences were statistically significant( P 〈0. 05). Conclusion The combined detection of Cys-C,TRF and NAG has important clinical significance in the early diagnosis of diabetic nephropathy.
