玉屏风颗粒对过敏性紫癜患儿Th17/Treg细胞失衡的调节作用 被引量:5

摘要 目的观察过敏性紫癜(HSP)患儿外周血Th17细胞及其相关因子IL-17和Treg细胞的水平,探讨玉屏风颗粒在调节HSP患儿Th17/Treg细胞失衡的作用以及治疗HSP的疗效。方法将确诊为过敏性紫癜初发的50例儿童,随机分为常规治疗组和玉屏风治疗组,各25例,疗程2周。25名同年龄段儿保科健康体检儿童为健康对照组。分别抽取健康对照组、常规治疗组和玉屏风治疗组治疗前后静脉血,采用流式细胞术检测外周血CD^(4+)CD^(25+)调节性T细胞(Treg)和Th17细胞比例;ELISA法检测血浆中IL-17水平。结果常规治疗组总有效率为68%,玉屏风治疗组总有效率为96%,2组比较有显著性差异(P<0.05)。常规治疗组和玉屏风治疗组患儿外周血Th17细胞数及其相关因子IL-17水平较健康对照组明显增高(均P<0.05),Treg细胞数较健康对照组明显降低(均P<0.05),但常规治疗组患儿Th17、Treg细胞数和IL-17水平与玉屏风治疗组患儿比较差异均无统计学意义(均P>0.05)。常规治疗组治疗后Th17细胞数、IL-17水平较治疗前有所下降,Treg细胞数较治疗前有所升高,但差异均无统计学意义(均P>0.05)。玉屏风治疗组治疗后Th17细胞数、IL-17水平较治疗前和常规治疗组治疗后明显下降(P<0.05),Treg细胞数较治疗前和常规治疗组治疗后明显增高(P<0.05)。结论 HSP患儿存在显著的Th17细胞及IL-17水平增高和Treg细胞减少。玉屏风颗粒能在一定程度上降低Th17细胞及IL-17水平,升高Treg细胞水平,增加HSP的疗效。 Objective To observe the levels of Th17 cells and associated cytokine IL-17 and Treg cells in peripheral blood in children with Henoch-Schonlein purpura, and to explore the role of Yupingfeng granule to regulate the imbalance of Th17/Treg and its clinical efficacy in children with HSP. Methods Fifty children diagnosed with HSP were randomly divided into routine treatment group and Yupingfeng treatment group. The number of each group is 25 children, and treatment for 2 weeks. Besides, another 25 healthy children in the same age were served as healthy control group. Peripheral vein blood was collected from three groups' children respectively before and after the treatment. Flow cytometry was used to detect proportions of Treg cells and Th17 cells in the peripheral blood. The enzyme linked immunosorbent assay(ELISA) was used to detect the level of IL-17. Results The total effective rate of routine treatment group was 68%, and the total effective rate of Yupingfeng treatment group was 96%. There was significant difference between the two groups(P〈0.05). The levels of Th17 cells and IL-17 in routine treatment group and Yupingfeng treatment group were significantly higher than those in healthy control group(all P〈0.05), while the level of Treg cells were lower than those in healthy control group( P〈0.05).However, there was no significant difference in the levels of Th17, Treg cells and IL-17 between the routine treatment group and Yupingfeng treatment group(all P〉0.05). The levels of Th17 cells and IL-17 in routine treatment group after treatment were lower than before, and the levels of Treg cells was higher than before, but there was no significant difference(all P〉0.05). The levels of Th17 cells and IL-17 in Yupingfeng treatment group after treatment were significantly lower than before and those in routine treatment group(all P〈0.05), but the level of Treg cells was higher than before and routine treatment group(all P〈0.05). Conclusion For HSP children,the levels of Th17 cells and IL-17 increase obviously, while the level of Treg cells decrease obviously. Yupingfeng treatment granule can decrease the levels of Th17 cells and IL-17, and increase the level of Treg cells, thus strengthen the efficacy of HSP.
