老年男性缺血性脑卒中后睡眠障碍相关危险因素分析

Risk factors of sleep disorders in male elderly patients after ischemic stroke

目的探讨老年男性缺血性脑卒中后出现睡眠障碍的相关危险因素,为临床预防及治疗提供依据。方法回顾性分析自2010年1月至2014年12月沈阳军区总医院干二科收治的550例65岁以上男性缺血性脑卒中患者的临床资料。采用匹茨堡睡眠质量指数量表(PSQI)评分、神经功能缺损程度(NFDS)评分及症状自评量表(SCL-90)评分对患者进行测评。应用Pearson单因素分析及Logistic回归分析对相关因素进行分析。结果本研究中550例脑卒中患者中,235例(42.7%)发生睡眠障碍。Pearson单因素分析显示,患者出现睡眠障碍与烟酒史、高血压、冠心病、糖尿病、NFDS评分及SCL-90评分有关(P<0.05)。Logistic回归分析显示,高血压、糖尿病、冠心病、中重型神经功能缺损、心理问题是缺血性脑卒中后发生睡眠障碍的危险因素(P<0.05)。结论老年男性缺血性脑卒中后发生睡眠障与糖尿病、冠心病、神经功能及精神状态等多种因素相关。

Objective To investigate the risk factors of sleep disorders in cadre wards of male elderly patients after ischemic stroke,and provide evidence for clinical treatment and prevention. Methods A retrospective study was performed on 550 cases of male elderly patients who were older than 65 years old with ischemic stroke from January 2010 to December 2014. The patients were assessed with Pittsburgh sleep quality index （PSQI）score, neurological deficit （NFDS）score and symptom checklist 90 （SCL-90） score. The correlation factors were analyzed by Pearson single factor analysis and Logistic re- gression analysis. Results Among the 550 cases of patients with ischemic stroke,235 cases（42.7% ） had sleep disorders. The Pearson univariate analysis showed that smoking and alcohol history, hypertension, coronary heart disease, diabetes mel- litus, NFDS scores and SCL-90 scores were found in patients with sleep disorders （P 〈 O. 05 ）. Then multiple regression anal- ysis showed that patients with hypertension, diabetes, coronary heart disease, neurological function and mental state were the risk factors after ischemic stroke for sleep disorders. Conclusion There are many factors related to the post-stroke sleep im- pairment in elderly men with ischemic stroke.such as diabetes.coronary heart disease.neuroloa＇ical function and mental state.
