四肢骨折并创伤性休克58例分析

Analysis of 58 cases of limb fracture with traumatic shock

目的观察四肢骨折并创伤性休克的临床治疗方法和治疗效果。方法选择2015年6月-2016年12月收治的四肢骨折并创伤性休克患者58例,回顾性分析其临床资料,总结患者的治疗方法和治疗效果。结果所有患者经过创伤性休克针对性治疗后,休克症状均得到改善,患者复苏后创伤严重程度(ISS)评分、输液量、输血量均高于复苏前,差异均有统计学意义(P<0.05)。所有患者经过骨折针对性治疗后,骨折症状均得到改善,其中解剖复位124处,功能复位44处,患者四肢骨折位置均得到复位。入院3 d内施行手术患者治疗有效率高于入院3 d后施行手术患者,不良反应发生率低于3 d后施行手术患者,术后住院时间短于入院3 d后施行手术患者,差异均有统计学意义(P<0.05)。结论早期针对性治疗能够有效改善四肢骨折并创伤性休克患者的术后恢复效果,明显提高患者临床治疗有效率,值得在临床治疗中大力推广和应用。

Objective To investigate the clinical treatment and therapeutic effect of limb fracture with traumatic shock. Methods 58 cases of limbs in June 2015 to December 2016 to the hospital for treatment of patients with fracture complicated with traumatic shock as the research object,a retrospective analysis of clinical data of the 58 patients,summarize the treatment methods of fracture and traumatic shock patients. Results All patients after traumatic shock targeted treatment,shock symptoms were improved,the scores of trauma severity（ ISS）,infusion volume and blood transfusion were significantly higher than those before resuscitation（ P 〈 0. 05）. All patients after fracture treatment,the fracture symptoms were improved,including anatomical reduction of 124,functional reduction of 44,the location of patients with limb fractures were reset. The effective rate of patients who underwent surgical treatment early（ 3 days） was higher than that of patients who were not treated early（ after 3 days）,the incidence of adverse reactions was less than that of patients who were not treated early（ after 3 days）,the hospitalization time was shorter than that of the patients who were not treated early（ after 3 days）,The differences were statistical significance（ P 〈 0. 05）. Conclusion Early targeted treatment of fractures and shock treatment can effectively improve the recovery of patients after surgery,significantly improve the efficiency of clinical treatment of patients,it is worthy of promotion and application in clinical treatment.
