改良耳软骨切取技术的临床应用探讨

The Clinical Application of Optimized Harvest Method of Auricular Cartilage

目的:探讨鼻整形中耳软骨切取的一种改良方法,既不遗留明显瘢痕,又可保持耳廓形状,还能最大限度地获取耳软骨。方法:2015年1月-2016年10月行鼻整形手术切取耳软骨患者148例,均单侧切取耳甲腔及耳甲艇软骨,140例右耳,8例左耳;选择耳甲艇和耳甲腔两个软骨供区,采用分段式切口,耳前对耳轮内侧切口切取耳甲艇软骨和对耳屏内侧切口切取耳甲腔软骨,切取时保留耳甲腔与耳甲艇交界区软骨等支撑结构。结果:148例受术者耳廓切口均一期愈合,未出现血肿、感染等并发症。经过1~10个月随访,耳廓外形基本不变,无明显切口瘢痕。结论:分段式内侧小切口切取耳软骨,保留耳甲腔与耳甲艇交界区软骨,有效保持了耳廓形状,术后瘢痕隐蔽,并发症少。

Objective To discuss a novel method to harvest the auricular cartilage in rhinoplasty by taking auricular cartilage maximally without obvious scar or auricle shape destroy. Methods From January of 2015 to October of 2016,148 cases with rhinoplasty of obtaining cavitas conchae cartilage and the cymba conchae cartilage were included. All cases were obtained on one side of auricular cartilage, 8 on the left side and 140 on the right side. Sectional incisions were performed for obtaining cavitas conchae cartilage and cymba conchae cartilage, with the integrity of auricle cartilage between the junction zones of them. Results All the 148 cases healed without complication of hematoma or infection. The follow-up was from 1 to 10 months. There were no visible scars or no visible transformation of auricle and antitragus. Conclusion Sectional incisions leave hidden cicatrices, reserve the junction zone of cavitas conchae cartilage and cymba conchae cartilage, and hold the structure of auricle with minimum complications.
