摘要
目的探讨磨痂术对烫伤大鼠术后创面铜绿假单胞菌感染的干预作用。方法 48只雄性Wistar大鼠随机分为A组和B组,每组24只,A组大鼠制造烫伤模型1~2小时后对创面实施磨痂术,术后涂铜绿假单胞菌标准菌株;B组大鼠制造烫伤模型1~2小时后对创面实施削痂术,术后涂铜绿假单胞菌标准菌株;两组实验动物于伤后3、5、7天各随机处死大鼠8只,左心室采血进行细菌培养,取创面组织标本进行HE染色及组织匀浆细菌定量培养。结果 A组血培养铜绿假单胞菌总阳性率明显低于B组,经比较差异有统计学意义(P<0.05);组织匀浆细菌定量培养统计结果显示,A组细菌计数明显少于B组,经比较差异有统计学意义(P<0.01);HE染色结果可见,两组随着受伤时间的延长,炎症程度明显加重,B组炎症程度重于A组,A组创伤组织侵袭性感染率明显少于B组,经比较差异有统计学意义(P<0.05)。结论磨痂术是早期处理烫伤深Ⅱ度创面的一种更为合理的手术方式。
OBJECTIVE To explore the interventional effect of woundabrasion on scalded rats with postoperative wound infected with Pseudomonas aeruginosa.METHODS Totally 48 male Wistar rats were randomly divided into the group A and the group B,with 24 rats in each group.The scalded rats models were established in the group A,then the wounds were treated with woundabrasion 1~2 hours later,the P.aeruginosa standard strains were coated after the surgery;the scalded rats models were established in the group,then the wounds were treated with cutting scab surgery 1~2 hours later,and the P.aeruginosa standard strains were coated after the surgery.Eight rats were randomly put to death in the two groups after they were scalded for 3,5 and 7 days,the left ventricular blood specimens were collected for bacterial culture,the wound tissue specimens were sampled for HE staining and tissue homogenate quantitative bacterial culture.RESULTS The positive rate of blood culture of P.aeruginosa was significantly in the group A than in the group B(P〈0.05).The result of tissue homogenate quantitative bacterial culture showed that the bacterial colony counts of the group A were significantly less than those of the group(P〈0.01).The result of HE staining indicated that the degree of inflammation was significantly aggravated with the time of injury,the degree of inflammation of the group B was more severer than that of the group A,and the inci-dence rate of invasive infection of wound tissues of the group A was significantly lower than that of the group B, and there was significant difference(P〈0.05).CONCLUSION The woundabrasion is a more reasonable surgical procedure for early treatment of deep grade II scald.
